摘  要:目的探讨微创碎吸术对大鼠脑出血后血肿周围脑组织磷酸化c-Jun氨基末端激酶(pJNK)和基质金属蛋白酶9(MMP9)表达的影响。方法成年健康雄性Wistar大鼠36只,随机分成假手术组、脑出血对照组和治疗组,每组12只。脑出血对照组及治疗组采用自体动脉血70μL脑内注射的方法建立脑出血模型,假手术组不注血。治疗组于造模后6h行微创碎吸术清除颅内血肿,脑出血对照组与假手术组不予抽吸治疗,自然恢复。所有大鼠均于造模后72h处死,用干湿质量法检测脑组织含水量,免疫组化法检测pJNK和MMP9的表达水平。结果大鼠脑组织含水量以脑出血对照组为最高,其次为治疗组,假手术组最低,3组比较差异有显著性(F=223.90,P<0.05);pJNK和MMP9表达水平均以脑出血对照组为最高,其次为治疗组,假手术组最低,3组比较差异均有显著性(F=98.73、203.40,P<0.05)。结论微创碎吸术可以降低脑出血后血肿周围脑组织pJNK和MMP9的表达水平,降低脑组织含水量,发挥脑保护作用。Objective To explore the effects of minimally invasive hematoma aspiration (MIHA) on the expressions of phospho e Jun NH2-terminal kinase (pJNK) and matrix metalloproteinase 9 (MMP9) after intraeerebral hemorrhage (ICH). Methods In this experiment,36 adult male Wistar rats were equally randomized to sham-operation group, control (ICH) group and treatment (M1HA) group. A model of ICH in rats was created by injection of 70 μL of autologous arterial blood into rat brains in ICH and MIHA groups, and those in sham-operation group were not injected, Six hours after creation of the model, MIHA was performed to remove intraeranial hematoma in rats in MIHA group, and those in sham-operation and ICH groups were untreated. All the rats were sacrificed 72 h after modeling. Water content of brain tissue wax detected using wet and dry mass method, and the expressions of pJNK and MMP9 were detected using immunohistochemistry method. Results The highest of water content in brain tissue was noted in ICH group, followed hy MIHA group, the lowest was seen in sham-operation group, the difference in wa ter content among the three groups was significant (F=223.90,P〈0.05). The highest expressions of pJNK and MMP9 were observed in ICH group, followed by MIHA group, the lowest was noted in sham-operation group, the difference hetween them was significant (F=98.73,203.40;P〈0.05). Conclusion Minimally invasive hematoma aspiration reduces the expressions of pJNK and MMP9 in brain tissue around hematoma after cerebral hemorrhage and water content in brain tissue, and thus protects the brain.
