2种手术方式治疗早期乳腺癌的远期效果研究

摘　　要：目的比较乳腺区段切除术并腋淋巴结清扫与改良根治术治疗临床Ⅰ、Ⅱ乳腺癌的远期临床效果。方法临床Ⅰ、Ⅱ乳腺癌患者100例,根据治疗方法分为乳腺癌改良根治术(对照组)及乳腺区段切除并腋淋巴结清扫术(观察组)各50例。比较2组术后生存率、转移率、复发率、体力状况评分以及生存质量评分。结果治疗后2组1年及3年生存率、复发率以及转移率比较差异均无统计学意义(P>0.05);2组体力状况评分均呈增高趋势,观察组增高幅度大于对照组,2组生存质量评分均呈下降趋势,观察组降低幅度小于对照组,组间、不同时点间、组间·不同时点间交互作用差异均有统计学意义(P<0.05)。结论乳腺区段切除并腋淋巴结清扫治疗早期乳腺癌远期疗效优于乳腺癌改良根治术,值得临床推广。Objective To analysis the long-term clinical results of modified radical mastectomy versus partial mastectomy with axillary lymph node dissection on phaseⅠ,Ⅱ breast cancer patients.Methods Breast cancer patients in phaseⅠ,Ⅱ（n=100）were divided into two groups：modified radical mastectomy group（control group,n=50）and partial mastectomy with axillary lymph node dissection group（observation group,n=50）.Survival rate,transfer rate,recurrent rate,performance status and scores of survival quality between the two groups were compared.Results There were no significant difference between the two groups in terms of survival rate,transfer rate and recurrent rate between the two groups（P 〉0.05）.There is an increasing trend in the performance status in the two groups,and the increase degree in observation groups is higher than control group.Scores of quality of life of the 2groups were all decreased.The observation group was lower than the control group and the descend range in the observation group is smaller than control group.There were statistically significant differences in the interaction of the two groups at the same time between the two groups,the difference between the two groups,and the difference between the two groups（P〈0.05）.ConclusionBreast section excision and axillary lymph node dissection in the treatment of early breast cancer is superior to breast cancer modified radical mastectomy.It is worthy of clinical application.
