摘　　要：目的总结肺硬化性血管瘤(pulmonary sclerosing hemangioma,PSH)的临床资料,提高对PSH影像学表现的认识。方法回顾分析本院1994年3月至2012年3月确诊的43例PSH患者的临床及CT资料,将CT表现与病理学结果进行对比分析。结果PSH多见于中年女性,临床表现缺乏特异性。PSH患者CT多表现为肺内孤立性肿块或结节影,密度均匀,可伴有分叶、点状钙化;可见相对特殊征象:空气新月征和贴边血管征。增强扫描强化程度主要与病理组织成分有关,均匀强化组为血管瘤区、乳头状突起、实体及硬化4种成分均匀分布,或以血管瘤样型和乳头状为主。不均匀强化组为上述4种成分不均匀分布,以硬化和实性为主,并伴有玻璃样变和钙化。无强化组分别为陈旧性出血和硬化为主。结论 PSH多发于中年女性,增强CT可见明显强化,强化程度取决于病理组织成分。Objective To summarize the clinical data of pulmonary hemangioma and to improve the understanding of pulmonary sclerosing hemangioma （PSH） imaging findings.Method The clinical and CT data of 43 PSH patients diagnosed in our hospital from March 1994 to March 2012 were analyzed retrospectively, and the CT findings and pathological results were compared and analyzed.ResultPSH was found mostly in middle-aged women and there was a lack of specificity in clinical symptoms of PSH. It showed solitary pulmonary mass or nodules which density was uniform and accompanied by leaf, punctate calcification in CT of PSH patients, and there were some visible relatively special signs： air crescent sign and welt vessel syndrome. CT enhanced degree of PSH were related to the pathological morphological characteristics. There was either composed of a uniform proportion of all four patterns, or a mixture of hemorrhagic and papillary patterns in the homogeneous density group. Heterogeneous density contained uneven proportion of all four patterns, with sclerotic and solid most frequently, accompanied by hyaline changes and calcification. Non-enhancement contained scleric and hemorrhagic patterns.Conclusion PSH occurs mostly in middle-aged women,the CT value obviously enhances. CT enhanced degree depends on the pathological morphological characteristics.
