结肠镜检查前不同肠道准备方法的临床效果评价

摘　　要：目的探讨结肠镜检查前不同肠道准备方法的临床效果。方法选择2013年6月—2015年6月在海军总医院消化内镜中心行结肠镜检查的患者180例,随机分为A、B、C 3组,每组60例。A组在检查前1晚及检查当日服用50%硫酸镁进行肠道准备,B组在检查前1晚及检查当日服用复方聚乙二醇电解质散进行肠道准备,C组在B组肠道准备的基础上在检查前1 h服用西甲硅油。观察3组结肠镜检查时肠道准备结果,包括肠道清洁程度、肠道气泡情况及患者不良反应发生情况。结果所有患者均完成肠道准备,结肠镜检查到达回盲部,B、C组结直肠息肉检出率高于A组(P<0.05)、肠道清洁度明显优于A组(P<0.05),C组肠道气泡量最少,图像清晰率明显高于A、B组(P<0.05)、不良反应发生率明显低于A、B组(P<0.05)。结论联合应用复方聚乙二醇电解质散和西甲硅油可提高结肠镜检查患者肠道清洁度,祛除肠道内气泡提高图像清晰度,减少不良反应,有利于内镜医师操作,从而提高肠道疾病检出率。Objective To investigate the clinical effects of different bowel preparation methods before colonoscopy.Methods A total of 180 patients undergoing colonoscopy during June 2013 and June 2015 were randomly divided into group A,B and C（ n = 60 for each group）.Group A was treated with 50% Magnesium Sulphate orally for bowel preparation at night before the colonoscopy;group B was treated with Polyethylene Glycol Electrolyte Powder orally for bowel preparation at night before the colonoscopy and on the day of colonoscopy;group C was added with Simethicone at1 h before the colonoscopy based on group B treatment.In the three groups,bowel preparation results including degree of intestinal cleanliness,conditions of intestinal bubbles and adverse reactions were observed.Results All patients finished bowel preparation,and colonoscopy reached the patient＇s ileocecal junction,and detection rates of colorectal polyps were higher（ P〈0.05）,and degree of intestinal cleanliness in group B and C were much better than those in group A（ P〈0.05）.In group C,amount of intestinal bubbles was the least one,and the image clarity rate was significantly higher（ P〈0.05）,and incidence rate of adverse reactions was significantly lower than those in group A and B（ P〈0.05）.Conclusion Polyethylene Glycol Electrolyte Powder combined with Simethicone in application of bowel preparation can improve intestinal cleanliness,remove intestinal bubbles,improve image clarity and decrease the adverse reactions,and it is conducive to endoscopic operation so as to improve the detection rate of intestinal diseases.
