脑干听觉诱发电位诊断小脑桥脑角肿瘤的探讨

摘　　要：报告12例小脑桥脑角肿瘤(CPAT)的BAEP表现,与32例耳蜗性聋对比,探讨了有关问题。本组以中、小型肿瘤为主,3个主波的检出率为33.3％,1或2个主波检出率为58.3％,各波均消失的为8.3％,可供潜伏期分析的占83％。各项潜伏期参数中,Ⅰ～ⅤIPL最可靠,经过校准的Ⅴ波ILD仍是重要的诊断指标,Ⅰ～Ⅲ波IPL及Ⅲ波ILD的实际应用价值有待积累更多的资料。有Ⅰ波而其余各波消失是CPAT的特征性波形;无反应波形只有结合纯音听力才有诊断意义。Brain-stem auditory evoked potentials (BAEPs)from 12 patients with cerebello-pontine angle tumor (CPAT) were analyzed by comparison with those from 32 patients who had cochlear hearing loss.BAEPs with all wave(Ⅰ,Ⅲ,Ⅴ) appeared in 33.3% of the patients with CPAT.BAEPs with one or low waves,in 58.3% of CPAT,in 8.3% of the cases,all the waves of BAEPs diasappeared.The absolute latency and interwave latency could be determined in 83% of this series of CPAT,among the parameters of the latency,the interval latency of Ⅰ-Ⅴ waves may be the most reliable parameter for the detection of CPAT.The Ⅴ ILD corrected might be a valuable index.But in clinical practice the values of wave Ⅰ-Ⅲ IPL and Ⅲ ILD might be questionable.It was shown that the BAEPs only with wave I was the characteristic of CPAT,and BAEPs with all waves absence should be estimated with audio-gram shape.
