腹腔镜手术联合药物治疗子宫内膜异位合并不孕症的效果观察

摘　　要：目的：研究子宫内膜异位症（endometriosis,EMT）合并不孕患者的近期妊娠情况。方法：选择2011年7月至2013年7月在我院治疗的EMT合并不孕患者272例作为研究对象。所有患者均进行腹腔镜手术,术后根据治疗方案对患者进行分组,其中孕三烯酮（gestrinone,GES）组86例,醋酸曲普瑞林（triptoelin acetate,TA）组90例,剩余96例作为对照组,统计并对比各组EMT合并不孕的治疗效果及各组不同分期EMT合并不孕的治疗情况。结果：治疗后,GES组和TA组的受孕率均显著高于对照组,且TA组的受孕率高于GES组,差异均有统计学意义（均P〈0.05）。GES组、TA组Ⅰ~Ⅱ期的受孕率与对照组相比,差异无统计学意义（P〉0.05）。GES组、TA组Ⅲ~Ⅳ期的受孕率显著高于对照组,差异有统计学意义（P〈0.05）。结论：腹腔镜手术联合药物治疗EMT合并不孕具有更好的疗效,值得临床重视。Objectives： To study the recent pregnancy situation of patients with endometriosis（ EMT） and infertility after treatment. Methods： 272 infertile patients with EMT treated in our hospital from July 2011 to July2013 were selected. All patients were given laparoscopic surgery,and according to the treatment,were divided into three groups,including the Gestrinone（ GES） group of 86 cases,the Triptoelin Acetate（ TA） group of 90 cases,and control group of 96 cases. Treatment effect of patients with EMT with infertility between groups and treatment condition of groups of different stages were compared and analyzed. Results： After treatment,the conception rate of GES and TA group was significantly higher than that of the control group,and TA group＇s conception rate was higher than the GES group,all with statistical significant difference（ P〈0. 05）. GES,TA group＇s Ⅰ ~ Ⅱ period of pregnancy compared with the control group,there was no statistically significant difference（ P〉0. 05）. GES,TA group＇s Ⅲ ~ Ⅳ period of pregnancy was significantly higher than the control group,with statistically significant difference（ P〈0. 05）. Conclusion： Laparoscopic surgery combined with drug therapy has better curative effect for EMT patient with infertility,which is worthy of clinical attention.
