窄谱中波紫外线联合阿维A治疗早期蕈样肉芽肿疗效观察

摘　　要：目的：观察窄谱中波紫外线（NB-UVB）单独或联合阿维A治疗早期覃样肉芽肿（mycosis fungoides,MF）的疗效。方法：61例I A期~ⅡA期MF患者随机分为两组,其中单纯照射组31例,应用NB-UVB单独照射;联合组30例,NB-UVB照射联合阿维A治疗。结果：61例患者平均治疗时间4个月,单纯照射组31例中,完全缓解11例,部分缓解13例,无效7例;联合组30例中,完全缓解14例,部分缓解12例,无效4例。联合治疗效果显著优于单独治疗,差异有统计学意义（P=0.0305）。联合组30例共接受照射次数729次,人均（24.3±7.5）次,单纯照射组31例共接受照射874次,人均（28.2±6.9）次。两组相比,联合组照射次数少于单纯照射组,差异有统计学意义（P〈0.05）。联合组30例人均照射剂量（27.69±13.58）J/cm2,单纯照射组31例人均照射剂量（30.45±11.27）J/cm2,联合组人均照射剂量少于单纯照射组,差异有统计学意义（P〈0.05）。两组均未出现严重不良反应。结论：联合治疗用于早期MF,疗效显著,优于单独照射,值得临床推广。Objective： To evaluate the therapeutic efficacy of narrow band ultraviolet B （ NB UVB ） alone or in combination with Acitretin for mycosis fungoides （MF）. Methods： Sixty-one patients with stage IA to ⅡA MF were ran- domly divided into two groups ： NB-UVB group （ n = 31 ） receiving NB - UVB Acitretin radiation only, combined group （ n = 30 ） treated with NB-UVB radiation plus Acitretin radiation. Statistic analysis was carried out by using t test, one - way a- nalysis of variance and Fisher＇ s exact test. Results ： The average treatment duration was 4 months. In NB-UVB group, 11 patients achieved complete remission, 13 partial remission,7 showed no response, while in the combined group, 14 experi- enced complete remission, 12 partial remission, and 4 showed no response. Therapeutic outcomes were significantly better in the combined group than in the NB-UVB group（P = 0.0305 ）. Patients in combined group received a total of 729 times ir- radiation, with an average of （24.3 ±7.5） times per capita. Patients in NB-UVB group received a total of 874 times irra- diation, with an average of （ 28.2 ± 6.9 ） times per capita. The number of irradiation in the combined group was less than that in the NB-UVB group with statistically significant difference （P 〈 0.05 ）. The radiation dosage received per capita in the combined group was （27.69 ± 13.58） J/cm2, which was significantly lower than that of the NB-UVB group [ （ 30.45 ± 11.27） J/cm2 ] （ P 〈 0.05 ）. No severe adverse effect was observed. Conclusion ： The therapeutic effect of NB-UVB ra- diation combined with Acitretin is superior to that of NB-UVB radiation alone for MF.
