线粒体激素受体P43在甲状腺乳头状癌中的表达及其临床价值

摘　　要：目的探讨线粒体激素受体P43(MT-P43)所在片段在甲状腺乳头状癌组织及结节性甲状腺肿组织中的转录水平及其与甲状腺乳头状癌临床病理的关系。方法采用实时荧光定量PCR法(RT-PCR),在mRNA水平检测31例甲状腺乳头状癌和31例结节性甲状腺肿新鲜标本及其各自对应的病变旁组织中MT-P43mRNA的转录表达情况。结果结节性甲状腺肿组织和甲状腺癌组织MT-P43mRNA转录水平均较各自配对的病变旁组织明显上升。31例结节性甲状腺肿较其病变旁组织MT-P43 mRNA转录水平明显上升,其相对倍比关系为(3.26±0.43),差异有统计学意义(t=4.35,P=0.000)。31例甲状腺癌较癌旁组织MT-P43mRNA转录水平明显上升,其相对倍比关系为(4.04±0.19),差异有统计学意义(t=3.375,P=0.001)。甲状腺癌较结节性甲状腺肿MT-P43mRNA转录水平明显上升,其相对倍比关系为(5.52±0.30),差异有统计学意义(t=2.040,P=0.025)。MT-P43mRNA的转录水平与患者性别、年龄及肿瘤大小无关(P>0.05);而与患者淋巴结转移及临床病理分期有关(P<0.05)。结论 MT-P43mRNA转录水平上升可能对甲状腺恶性病变的发生有促进作用,转录水平越高,越容易发生恶变,这对于甲状腺癌的诊断及鉴别诊断有一定的提示价值。Objective To investigate the transcription level of mitochondrial hormone receptor（in fragments of MT-P43）in thyroid carcinoma tissue and nodular goiter tissue,and the relationship between the transcription level of P43 and clinical pathology of thyroid cancer.Methods MT-P43 mRNA transcription and expression in 31 cases of thyroid carcinoma and 31 cases of nodular goiter fresh specimens were detected by real-time fluorescent quantitative PCR（RT-PCR）.Results The transcription of MTP43 mRNA in the group of nodular goiter and thyroid cancer were higher than their counterparts of the adjacent normal tissue.MT-P43 mRNA transcription level raised obviously in 31 cases of nodular goiter,and it was（3.26±0.43）times the quantity of that in the adjacent normal tissue,and the difference was statistically significant（t=4.35,P =4.35）.The transcription level of MT-P43 mRNA raised obviously in 31 cases of thyroid carcinoma compared to that of the para-tumorous.The relative ratio of transcription level of MT-P43 mRNA was（4.04±0.19）between thyroid carcinoma tissue and their para-tumorous tissue,and the difference was statistically significant（t=3.375,P =3.375）.MT-P43 mRNA transcription level in thyroid carcinoma group rose more than that in the nodular goiter group.The relative ratio was（5.52±0.30）between those two groups,and the the difference was statistically significant（t=2.040,P =2.040）.MTP43 mRNA transcription level was not related to the patients′gender,age and tumor size（P〈0.05）;While it was associated with lymph node metastasis and clinical pathologic staging（P〈0.05）.Conclusion The raising of MT-P43 mRNA transcription levels may promoting malignancy of thyroid.The higher the level of transcription,the easier the thyroid tissue transform to malignantion,which is valuable for the diagnosis and differential diagnosis of thyroid carcinoma.
