牙周基础治疗对冠心病病人血清中Fg、hs-CRP和IL-6的影响分析

摘　　要：目的：通过分析冠心病伴慢性牙周炎病人牙周基础治疗前后牙周状况和血清中Fg、hs-CRP及IL-6水平的变化,探究牙周基础治疗对冠心病治疗效果的影响。方法：选取2012-12~2013-12在我院心血管内科确诊为冠心病并伴有慢性牙周炎病人64例为研究对象。随机分为实验组和对照组各32例,实验组给予心血管治疗及牙周基础治疗,对照组仅进行心血管治疗。比较治疗前、治疗后3mo两组牙周临床指标牙周袋深度（PD）、临床附着丧失（CAL）、龈沟出血指数（SBI）和血清中纤维蛋白原（Fg）、超敏C反应蛋白（hs-CRP）和白细胞介素-6（IL-6）水平的变化。结果：实验组治疗前后,牙周临床指标和血清中Fg、hs-CRP及IL-6含量比较,差异有统计学意义（P〈0.05）;对照组治疗前后比较,牙周临床指标无统计学意义（P〉0.05）,血清中Fg、hs-CRP及IL-6含量比较,差异有统计学意义（P〈0.05）;实验组和对照组自身治疗前后差值比较,牙周临床指标和血清中Fg、hs-CRP和IL-6水平明显降低,差异有统计学意义（P〈0.05）。结论：冠心病伴慢性牙周炎病人在心血管治疗基础上,进行牙周基础治疗可协助降低冠心病相关危险炎性因素水平,延缓冠心病病程发展,对冠心病的防治有积极作用。Objective： To analyze periodontal status and the level of serum Fg,hs- CRP and IL- 6before and after periodontal therapy in chronic periodontitis patients with coronary heart disease,and explore the influence of periodontal initial therapy for the treatment effect of coronary heart disease.Methods： 64 patients were selected in our hospital from 2012- 12 ~ 2013 ~ 12 diagnosed of coronary heart disease with chronic periodontitis. They were divided into two groups randomly,experimental group and control group,each group of 32 cases. Two groups were treated with same routine medicine therapy and related care,another experimental group given periodontal treatment. The serum levels changes of Fg、hs- CRP and IL- 6 and changes of PD、CAL、SBI,before and after the intervention were analyzed. Results： Statistically significant differences were found in clinical periodontal status and serum levels of Fg、hs- CRP and IL- 6 among experiment group before and after periodontal therapy（ P〈0. 05）. Obvious differences were found of the clinical periodontal status in experiment and control group,and greater improvement of inflammation situation（ serum Fg、IL- 6 and hs- CRP） was found in experiment group（ P〈0. 05）. Conclusion： The study demonstrates that on the basis of cardiovascular therapy,periodontal initial therapy can help decrease the serum markers levels such as Fg、hs- CRP and IL- 6 in chronic periodontitis patients with coronary heart disease,delay the course of the disease,and has a positive effect for the treatment and prevention of coronary heart disease.
