摘  要:目的:比较IOL Master与传统超声生物测量法对人工晶状体度数测量的准确性和特点,评价IOL Master的临床应用价值。方法:分析2014-06/2015-06间于我院行白内障超声乳化摘除及人工晶状体植入术的年龄相关性白内障患者164例206眼,术前分别用IOL Master、传统的超声生物测量仪和角膜曲率计测量眼轴长度和角膜曲率,使用SRK-Ⅱ公式计算人工晶状体度数。对患者施行超声乳化白内障摘除术后,按照IOL Master测量得出的人工晶状体度数植入可折叠人工晶状体,术后3mo检查患者的视力和屈光状态。结果:IOL Master和超声波检查法测得的眼轴分别为23.86±1.05mm和23.50±0.83mm,两者对比差异有统计学意义(P=0.025)。IOL Master和手动角膜曲率计测得的角膜曲率分别为44.18±1.35D和43.70±1.41D,两者对比差异有统计学意义(P=0.01)。术后3mo患者平均绝对屈光误差分别为0.41±0.30D和0.93±1.10D,两者对比差异有统计学意义(P=0.027)。结论:IOL Master具有非接触性、操作简便、安全而且易于被患者接受的特点,对眼轴长度和角膜曲率测量的精确性较高,从而对人工晶状体度数测算更精确,提高了对患者术后屈光状态的可预测性。AIM : To evaluate the clinical application of IOL Master by com paring with traditional ultrasound biom etry on the accuracy and characteristics of intraocular lens calculation.METHODS: Data was analyzed from 164 patients( 206eyes) with age-related cataracts who underwent phacoem ulsification and intraocular lens( IOL)im plantation in our hospital from June 2014 to June 2015.Before surgery,axial length and corneal curvature were m easured with IOL M aster and com bined application of ultrasonic or m anual keratom etry, respectively.Phacoem ulsification and foldable lens im plantation were done in the patients. IOL power calculation was carried out using the SRK-Ⅱ form ula with the basis of IOL M aster data. The visual acuity and refractive outcom e were followed-up for 3m o postoperatively. RESULTS: There was a significant difference between the two m ethods on axial length m easurem ent which was23. 86 ± 1. 05 m m by IOL Master and 23. 50 ± 0. 83 m m by ultrasound( P = 0. 025). There was also a significant difference between the two m ethods on corneal curvature m easurem ent which was 44. 18±1. 35 D by IOL M aster and43. 70 ± 1. 41 D by keratom etry( P = 0. 01). The m ean absolute error( M AE),at 3m o after operation,was 0. 41±0. 30 D and 0. 93±1. 10 D by the IOL M aster and ultrasound groups,respectively,there was a significant difference between the two m ethods( P= 0. 027).CONCLUSION: The IOL Master is a non-contact,safe,easy-to-do and patient-friendly m ethods for axial lengthand corneal curvature m easurem ent with high accuracy,thus it can calculate the IOL power m ore accurate and im prove the predictive value for postoperative refraction.
