25G玻璃体腔灌注在玻璃体切除术后的白内障超声乳化手术中的应用

摘　　要：目的：探讨25G玻璃体腔灌注在玻璃体切除术后的白内障超声乳化手术中的应用效果。方法：选取2013-09/2014-09于我院诊治的经睫状体平坦部玻璃体切除术后白内障患者48例50眼,对其行预置25G玻璃体腔灌注下白内障超声乳化联合人工晶状体植入术,观察术后最佳矫正视力、眼压、术后不适、巩膜穿刺口愈合情况及术后并发症。结果：所有患者均顺利完成手术。与手术前相比,患者白内障手术后90d最佳矫正视力≤0.1的眼数较少,最佳矫正视力0.3~0.5、〉0.5、〉0.3的眼数较多,差异有统计学意义（P〈0.05）。1眼因为原有眼底病变复发引起玻璃体积血,视力较术前下降。术前、术后1、7d,6mo平均眼压分别为14.67±0.62、14.75±0.56、14.86±0.58、14.88±0.59mmH g。眼压在患者白内障手术前与术后1、7d,6mo间比较差异无统计学意义（P〉0.05）。术后无明显眼部不适及严重并发症。结论：25G玻璃体腔灌注应用于玻璃体切除术后的白内障超声乳化手术中对维持眼压、稳定前房及减少并发症有益处。AIM ： To explore the application of 25 G vitreous cavity perfusion in phacoem ulsification for cataract after vitrectom y.METHODS： Forty-eight patients（ 50 eyes） with cataract after pars planavitrectomy in our hospital from Septem ber2013 to Septem ber 2014 were selected to receive phacoem ulsification and im plantation of artificial lens with25 G vitreous cavity perfusion. Best corrected visual acuity（ BCVA）, intraocular pressure（ IOP）, postoperative discom fort,healing of scleral puncture and postoperative com plications were observed.RESULTS： All patients underwent surgery successfully.Com pared to before operation,there was less patients with BCVA ≤0. 1,m o r e p atien ts with BCVA 0. 3-0. 5,0. 5,0. 3（ P0. 05）. The visual acuity of 1 eye decreased because of vitreous hem orrhage caused by original fundus lesion recurrence. The m ean IOP before operation and at 1,7d,6m o after operation were 14. 67±0. 62 m m Hg,14. 75±0. 56 m m Hg,14. 86±0. 58 m m Hg,14. 88±0. 59 m m Hg,respectively. There were no statistically significant differences of IOP between that before operation and those at 1,7d,6m o after operation（ P0. 05）. No obvious eye discom fort and severe com plications after operation were observed.CONCLUSION： The effect of 25 G vitreous cavityperfusion in phacoem ulsification for cataract after vitrectomy is good and it can m aintain IOP,keep anterior cham ber stable and decrease the com plications risk.
