甲状腺微小乳头状癌的MRI术前评估分期与术后病理对照研究

摘　　要：目的:分析甲状腺微小乳头状癌的MRI术前评估分期与术后病理对照。方法:选取于2012年9月-2014年9月在本院治疗的甲状腺微小乳头状癌患者59例作为观察组,选取同期入院体检正常者30例作为对照组,分析两组患者MRI颈静脉造影血液动力学改变情况。结果:观察组的血管内径、血流量参数较对照组低,平均血流速度较对照组快,差异均有统计学意义(P<0.05)。对比分析不同TNM分期患者的甲状腺微小乳头状癌,T期患者血管内径明显大于N、M期,平均血流速度低于N、M期,差异均有统计学意义(P<0.05)。三组患者血流量参数的变化情况比较差异无统计学意义(P>0.05)。结论:加强MRI颈静脉造影血液动力学改变在甲状腺微小乳头状癌预测中的应用,能够了解患者疾病发展情况,对临床确定手术方案具有较高的应用价值。Objective：To analyze pathologic control over preoperative staging assessment and postoperative staging of pre-operative MRI thyroid papillary carcinoma.Method：Fifty nine patients with thyroid papillary carcinoma treated in our hospital from September 2012 to September 2014 were divided into the observation group,and 30 subjects confirmed to be normal via physical examination were divided into the control group,and the changes in MRI jugular vein angiography hemodynamics of patients in two groups were analyzed.Result：Internal vessel diameters and blood flow parameters of patients in the observation group were lower than those of patients in the control group,while average blood flow rate of patients in the observation group was faster than that of patients in the control group,and the differences were statistically significant（ P〈0.05）.The thyroid papillary carcinomas of patients at different Child-Pugh stages were compared and analyzed,the internal vessel diameters of patients with grade A were significantly higher than that of grade B and grade C,average blood flow rate of patients at grade A was lower than that of patients at grade B and grade C,and the difference were statistically significant（P〈0.05）.The differences of changes in blood flow parameters and inter-group had no statistical significance（P〈0.05）.Conclusion：Enhanced application of changes in MRI jugular vein angiography hemodynamics of patients with thyroid papillary carcinoma forecast may understand development conditions of diseases,casting high value clinically applied in determining the operation solution.
