摘 要:目的了解我国"农村义务教育学生营养改善计划"(以下简称"学生营养改善计划")试点地区学生维生素D营养状况,为进一步改善我国贫困农村儿童营养健康状况提供基础数据。方法从"学生营养改善计划"覆盖的22个省(直辖市、自治区、新疆建设兵团)的50个重点监测县中,每县随机抽取小学和初中各2所,再从各年级抽取1-2个班的学生,每个年级抽取40名学生左右。2013年9—11月,共对10 760名5-19岁学生采集空腹静脉血,分离血清,用液相色谱串联质谱仪(LC-MS/MS)测定血清25(OH)D[含25(OH)D3和25(OH)D2]。结果学生血清25(OH)D平均水平为(20.7±11.0)μg/L,维生素D缺乏率[25(OH)D〈10μg/L]为12.3%,女生(14.3%)高于男生(10.2%)(χ^2=34.37,P〈0.01),初中生(16.5%)高于小学生(10.0%)(χ^2=47.71,P〈0.01)。维生素D不足率[25(OH)D为10-20μg/L]为42.0%,女生(44.8%)高于男生(39.2%)(χ^2=41.70,P〈0.01),初中生(46.5%)高于小学生(39.6%)(χ^2=95.48,P〈0.01)。结论 "农村学生营养改善计划"试点地区的农村中小学生维生素D营养状况较差,应采取合理措施改善维生素的营养状况。Objective To understand the status of vitamin D deficiency( VDD) and vitamin D insufficiency( VDI) among students from the National Nutrition Improvement Program for Rural Compulsory Students( NNIPRCS). Methods One elementary school and one middle school were randomly selected from 50 key monitoring counties, 1- 2 classes of students were randomly selected from each school. Fasting venous blood samples were collected and LC- MS / MS method was used to determine 25( OH) D concentration( including 25( OH) D3 and 25( OH) D2). Results Totally 10 760 students were enrolled in this study, 5 448 males and 5 312 females. The average 25( OH) D concentration was( 202. 7 ± 11. 0) μg / L. The prevalence of VDD was 12. 3%, female students( 14.3%) were more likely to get VDD than male students( 10.2%)( P〈0.01), students from elementary schools( 16.5%)were more likely to get VDD than students from middle schools( 10. 0%)( P 0. 01). The prevalence of VDI was 42. 0% in these students,female students( 44.8%) and middle school students( 46.5%) were more likely to get VDI than male students( 39.2%) and elementary school students( 39. 6%). Conclusion Vitamin D Deficiency remains to be a public health problem in pilot areas of NNIPRCS.
