42例眼眶内下壁爆裂性骨折Medpor修复疗效评价

摘　　要：目的探讨高密度多孔聚乙烯（Medpor）植入术治疗眼眶内下壁爆裂骨折的疗效。方法对确诊为眼眶内下壁爆裂性骨折患者42例（42只眼）行眼眶整复及骨折区骨膜下Medpor植入,术后随访1~12个月,观察比较术前术后眼球内陷、复视、眼球运动及视力等变化。结果术后所有患者眼球内陷症状均得到矫正,双眼突出度基本一致,相差均〈2 mm。38例复视消失,治愈率为90.5%,3例复视改善但仍存在,1例复视无改善。眼球各方向运动基本到位,手术未影响视力,术后随访期间无植入物脱出或排斥现象。结论眼眶内下壁爆裂性骨折使用Medpor植入体进行修复,恰当选择手术时机并合理运用手术技巧,有助于改善眼球内陷和复视。OBJECTIVE To study the effect of medpor implant to repair medial and inferior orbital wall in blowout fracture. METHODS Forty-two medial and inferior orbital wall blowout fracture patients received orbital reconstruction with medpor implantation. And they were followed up for 1-12 months for improvement of enophthalmos, diplopia, ocular movement and vision. RESULTS Enophthalmos in all cases were corrected.To be specific, the post-operative protrusion degree of both eyes was almost identica 1 in that the difference was less than 2 mm. Diplopia disappeared in 38 cases which meant cure rate was 90.5%.Three case were improved and 1 case showed no effects.The movement of eye balls was satisfactory in all directions.There was no instance of decreased visua1 acuity, neither implant shedding nor rejection after operation. CONCLUSIONS Medpor implant was a safe and effective treatment in repair of medial and inferior orbital wall blowout fracture.
