针刀与髓芯减压治疗早、中期股骨头缺血性坏死的临床随机对照研究

摘　　要：目的:比较针刀与髓芯减压治疗早、中期股骨头缺血性坏死的临床疗效。方法:选取60例Ficat分期I-II的股骨头缺血性坏死患者,用随机数字表法将其随机分为治疗组与对照组,并采用IBM SPSS 21 Statistics统计软件对两组Harris髋关节功能评分和总体有效率进行统计学分析。结果:治疗组与对照组治疗前Harris髋关节功能评分统计学分析t=0.79,P=0.46,P>0.05;两组治疗后Harris髋关节功能评分t=3.27,P=0.002,P<0.05;治疗组与对照组治疗前与治疗后的Harris髋关节功能评分统计学分析结果分别为t=12.53、t=11.35,P=0.00、P=0.00,P<0.05;治疗组与对照组两组治疗有效率进行统计学分析,结果为Z=2.12,P=0.02,P<0.05。结论:针刀和髓芯减压对股骨头缺血性坏死均有治疗作用,但针刀对股骨头缺血性坏死的作用优于单纯髓芯减压。所以针刀治疗股骨头缺血性坏死的方法建议在临床中推广应用。Objective： To compare the clinical curative effect of needle- knife and marrow core decompression treatment for early and middle avascular necrosis of the femoral head. Methods： Sixty cases of patients with avascular necrosis of the femoral head at stage I- II,according to the random number table method,were randomly divided into treatment group and control group.The IBM SPSS 21 Statistics statistical software was used to make the on statistical analysis on two groups＇ Harris hip joint function score and total efficient. Results： Before treatment,treatment and control groups＇ Harris hip score was not significant,t = 0. 79,P =0. 46,P 0. 05. After treatment,the two groups＇ Harris hip function score was significant,t = 3. 27,P = 0. 002,P 0. 05. Before and after treatment,treatment and control groups＇ Harris hip joint function scores were respectively significant,t = 12. 53,t =11. 35,P = 0. 00,P = 0. 00,P 0. 05. Two groups＇ efficient treatment results were significant,Z = 2. 12,P = 0. 02,P 0. 05. Conclusion： Both needle- knife and marrow core decompression have effect on avascular necrosis of femoral head,but the needle-knife has better effect,which is worthy of application in clinic.
