多模态影像融合技术在脑肿瘤手术中的应用

摘　　要：目的利用多模态影像数据配准融合和三维重建的方法构建脑肿瘤及其周围的解剖结构，旨在为脑肿瘤手术提供指导。方法选取2015年4月至2015年12月清华大学玉泉医院神经外科收治的10例脑肿瘤患者，采用头颅CT、MRI对脑肿瘤患者进行数据采集，利用NeuroTech软件对影像数据进行配准融合和三维重建。结果对10例脑肿瘤患者的颅骨、脑肿瘤、脑组织以及血管等组织的三维结构进行重建，可清晰显示肿瘤的大小、位置、形状以及肿瘤与周围结构的解剖关系，从而在手术切口设计、手术入路和肿瘤切除方式的选择等方面发挥辅助作用。结论采用多模态影像融合技术能直观地显示肿瘤与血管、脑组织、颅骨等组织的解剖关系，提供大量的可视化信息，指导神经外科手术的切口设计、手术入路的选择以及肿瘤切除，并在脑功能的保护、减少术后并发症方面发挥作用。Objective To construct brain tumors and their surrounding anatomical structures using the multi-modality image data registration and fusion and three dimensional reconstruction method in order to provide guidance for brain tumor surgery. Methods Ten patients with brain tumor admitted to the Department of Neurosurgery, Yuquan Hospital, Qsinghua University from April 2015 to December 2015 were selected. Head CT and MRI were used to collect the data in patients with brain tumor. NeuroTech software was used to conduct registration and fusion and 3D reconstruction for image data. Results The 3D structures of brain tumor, brain tissue, and blood vessel in 10 patients with brain tumor were reconstructed. They clearly revealed the anatomic relationship of tumor size, location, shape, and tumor and surrounding structures. They assisted inthe design of the surgical incision and the selection of operative approach and tumor resection. Conclusions Using multi-modality medical image fusion technology can visually display the relationship between tumors and blood vessels, brain tissue, skull, and other tissues, provide a large amount of visual information, guide the design of incision of neurosurgery, choose the operative approaches, resect the tumors, and play roles in the aspect of protecting brain function and reducing postoperative complications.
