摘 要:目的 观察非神经元型胆碱能系统(NNAS)在重症肌无力(MG)患者胸腺组织中的表达.方法 取MG患者及对照组胸腺组织,提取总RNA,通过实时定量聚合酶链反应(Real-time PCR)检测烟碱型乙酰胆碱受体(nAchR)亚型(α1、α2、α3、α4、α5、α6、α7、α9、α10、β1、β2、β3、β4、γ)、乙酰胆碱转移酶(ChAT)以及毒荨碱型乙酰胆碱受体(mAchR)亚型(m1、m2、m3、m4、m5)的表达;制作胸腺组织石蜡切片,通过免疫组织化学法了解nAChR、mAChR在胸腺组织中的分布与表达情况.结果 Real-time PCR结果显示MG患者胸腺组织α1、α9、m3的mRNA的表达水平分别为10.52±1.38、13.39 ±0.69、14.40±1.51,与对照组比较显著增高(P <0.05);α2、β2的mRNA表达水平分别为15.45±1.32、13.99±3.32,与对照组比较显著降低(P<0.05),α3、α5、α6、α7、α10、β1、β3、β4、、ChAT、m1、m2、m4、m5的mRNA表达水平与对照组比较差异无统计学意义(P>0.05).免疫组织化学显示α1、α9、m3、α2、β2在MG患者及对照组胸腺组织均有表达,主要分布在胸腺组织的胞质与胞膜,偶尔也在胞核中表达.MG患者胸腺组织α1的表达显著高于对照组,差异有统计学意义(P <0.05);MG患者胸腺组织α2、β2的表达显著低于对照组,差异有统计学意义(P <0.05);α9、m3在MG胸腺组织与对照组比较差异无统计学意义(P>0.05).结论 MG患者胸腺组织NNAS发生改变,为探讨MG的发病机制提供重要的理论基础.Objective This study aims to explore the expression of non-neuronal cholinergic system (NNAS) in the thymus of patient with myasthenia gravis (MG) by analyzing the expression of nicotinic acetylcholine receptors (α1-α10,β1-β4,γ),muscarinic acetylcholine receptors (m1-m5) and choline acetyltransferase (ChAT) in thymus of MG patients.Methods Total RNA extracted from the thymus tissue of MG patients and control people were collected,then the mRNA expression levels of nicotinic acetylcholine receptors (α1-α10,β1-β4,γ),muscarinic acetylcholine receptors (m1-m5) and ChAT were examined by real-time quantitative polymerase chain reaction (Real-time PCR).At the same time,paraffin-embedded sections of thymus from MG patients and control people were made and stained with Immunohistochemistry.Results The results of real-time PCR showed that the mRNA expression of α1,α9,m3 in thymus tissue of MG respectively are 10.52 ± 1.38,13.39 ± 0.69,14.40 ± 1.51,increased significantly as compared with those in thymus tissue of control people (P 〈 0.05);the mRNA expression of α2,β2 in thymus tissue of MG respectively are 15.45 ± 1.32,13.99 ± 3.32,reduce significantly as compared with those in thymus tissue of control people (P 〈 0.05);the results of immunohistochemistry showed that although α1,α9,m3,α2,β2 mainly distributed in the membrane and cytoplasm,expressed not only in the thymus tissue of MG patients but also control people.the protein expressions of α1,β2 were significantly higher in thymus tissue from MG patients than those from control people (P 〈 0.05).the protein expressions of α2 were significantly lower in thymus tissue from MG patients than those from control people (P 〈 0.05).Conclusion The change of NNAS in the thymus of patients with MG provides an important theoretical basis for the study of the pathogenesis of MG.
