真空辅助活检在乳腺导管内乳头状瘤治疗中的应用

摘　　要：目的：探讨真空辅助活检在乳腺导管内乳头状瘤治疗中的价值。方法：回顾分析2012年1月-2015年12月我科行超声引导下真空辅助活检治疗的1 210例乳腺疾病患者,其中经病理证实为导管内乳头状瘤者62例,通过对比、分析导管内乳头状瘤临床特点及回访,明确真空辅助活检在乳腺导管内乳头状瘤治疗中的价值。结果：手术时间10-40 min,平均15 min;术后6例出现并发症;中央型乳头状瘤与外周型乳头状瘤在发病年龄、肿块大小、超声BI-RADS分级等方面比较差异无统计学意义;术后随访2-36个月,中位随访18个月,乳腺外观良好,美容效果良好。临床查体结合彩色超声检查未发现原手术部位病灶残留或复发。结论：真空辅助活检是一种微创、有效的治疗乳腺导管内乳头状瘤的方法。Objective To investigate values of vacuum assisted core biopsy in breast intraductal papilloma.Methods In a period from 2012.01 to 2015.12, a total of 1210 vacuum assisted core biopsies were performed,of which 62 were confirmed intraductal papilloma by histopathological examination. In order to clarify the value of vacuum assisted core biopsy in treatment of intraductal papilloma, the clinical features were analysed. We also performed a return visit of the patients. Results The operation time were 10 to 40 mins（15 mins on average）.Six cases of postoperative complications were found. There were no significant differences in age, tumor size,and BI-RADS grade between central papilloma and peripheral papilloma. In all 62 cases with benign papilloma in biopsy specimens, further clinical observation（2-36 months, 18 months on average） did not show recurrence or malignant transformation of lesions. Conclusion Vacuum assisted core biopsy is a minimally invasive and effective method for treatment of breast in intraductal papilloma.
