肝动脉化疗栓塞术联合肿瘤边缘适形治疗肝癌患者的临床疗效

摘　　要：目的:探讨经肝动脉化疗栓塞术(TACE)联合经皮穿刺肿瘤边缘残留病灶注射碘油化疗乳剂的临床疗效。方法:78例TACE治疗后PET-CT或CT复查肿瘤边缘残留患者分为两组。联合组经TACE后加行经皮穿刺肿瘤残留边缘注入碘油化疗乳剂。对照组患者单纯采用TACE。治疗后观察、比较两组AFP水平、病灶完全坏死率,并随访分析生存率。结果:联合组的病灶完全坏死率为84%,对照组的完全坏死率为47%,差异有统计学意义(P<0.05)。联合组与对照组AFP总下降幅度比较差异有统计学意义(P<0.05)。联合组1、2年生存率为81%、52%。对照组1、2年生存率分别为68%、29%。两组第2年生存率比较差异有统计学意义(P<0.05)。结论:边缘适形治疗是原发性肝癌TACE术后边缘残留病灶的灭杀的一种有效手段,值得提倡。Objective To evaluate the therapeutic efficacy of transcatheter artery chemoembolization（TACE） combined with percutaneous intratumoral iniection of lipiodol emulsion in residual tumor edge. Methods79 cases with residual tumor edge after TACE by PET-CT or CT check were randomly divided into two groups. Patients in combination therapy group（group A,n = 39） were treated with TACE together with percutaneous intratumoral injection of lipiodol emulsion in residual tumor edge and patients in TACE group（group B,n = 39） were treated with TACE only. AFP level and the complete necrosis rate of tumor were reexamined after treatment. The survival rate was calculated. The results were compared between the two groups.Results The complete necrosis rate of tumor were 84% in group B,and 47% in groups A respectively.There were significant statistical differences between the two groups（P〈0.05）. The difference in AFP level reduction between the two groups was statistically significant（P〈0.05）.The one-year and two-year survival rates were 81%and 52% in group A respectively. The one-year and two-year survival rates were 68% and 29% in group B. The difference in terms of the two-year survival rates was statistically significant（P〈0.05）. Conclusions Conformal therapy is an effective method for elimination of residual tumor edge after TACE, worthy to be recommended.
