浅析对血管性痴呆患者进行团体心理治疗对其事件相关电位P300的影响

摘　　要：目的 :探讨对血管性痴呆患者进行团体心理治疗对其事件相关电位P300的影响。方法 :选取近期福建中医药大学附属康复医院收治的58例VD患者作为研究对象。根据随机数表法将这些患者分为对照组和团体治疗组,每组各29例患者。为两组患者均进行常规治疗,为团体治疗组患者在进行常规治疗的基础上进行团体心理治疗。然后比较两组患者治疗前后的Mo CA评分及P300潜伏期的长短和波幅的大小。结果 :治疗8周后,两组患者的Mo CA评分均得到了显著的提高,且团体治疗组患者提高的幅度明显大于对照组患者,差异显著,具有统计学意义(P<0.05);两组患者P300的潜伏期缩短,波幅升高,且团体治疗组患者潜伏期缩短的幅度和波幅升高的幅度均明显大于对照组患者,差异显著,具有统计学意义(P<0.05)。结论:对血管性痴呆患者在进行常规治疗的基础上进行团体心理治疗能显著改善其认知能力,缩短其P300的潜伏期,增大其P300的波幅。Objects To investigate the effects of Group Psychotherapy on the patients with vascular dementia(VD),and to observe the change in the event related potential(ERP). Methods 58 patients with VD were randomly divided into two groups : control group and group psychotherapy group. Each group have 29 patients. Both the control group and the group psychotherapy group were given the same conventional treatment. At the same time, the group psychotherapy group accept group psychotherapy: 8 weeks, 1 time/week, 1h/time, of treatment. The two groups, respectively, before and after 8 weeks of treatment, were assessed by Montreal cognitive assessment(Mo CA). And the ERP datas of both groups were recorded. Results The Mo CA scores: after 8 weeks, scores of both groups were improved(P <0.05), especially in the group psychotherapy group(P <0.01); ERP data: After 8 weeks, both groups have the similar P300 waveform differentiation. That is, latency of P300 was shortened, amplitude increased, with significant difference(P < 0.05). But compared with 8 weeks before, the P300 difference of group psychotherapy group was more significant(P < 0.05). Conclusion The conventional treatment alliance by group psychotherapy can significantly improve the cognitive function of patients with VD, and P300 can be used as an important index for the judgment of therapeutic effect.
