摘 要:目的:观察吸入高浓度氢气对大鼠全脑缺血再灌注(I/R)损伤海马CA1区神经元细胞及小胶质细胞的影响。方法:雄性SD大鼠75只,采用随机数字法分为3组,假手术(SH)组15只,采用电凝双侧椎动脉,仅暴露双侧颈总动脉但不夹闭;模型(4vo.con)组30只,用4血管阻断法(4-VO)建立全脑I/R损伤模型,再灌注同时吸入67%N_2,33%O_2;氢气治疗(4vo+H_2)组30只,造模后再灌注同时吸入67%H_2,33%O_2,3 d和5 d后,观察、分析海马CA1区神经细胞形态和数量。结果:模型组海马CA1区神经元排列疏松、明显变性、细胞核固缩,大量尼氏小体丢失。氢气治疗组海马CA1区神经元稍有疏松,细胞结构相对完整,尼氏小体细胞数量减少,病理学损伤较模型组明显减轻。氢气治疗3 d或5 d后,神经元数量比模型组多,但低于假手术(SH)组,差异有统计学意义(P<0.05);小胶质细胞数量多于假手术组,但低于模型(4vo.con)组,差异有统计学意义(P<0.05)。结论:再灌注同时吸入高浓度氢气对大鼠全脑I/R损伤海马CA1区神经元可产生明确保护作用,其机制可能是氢气抑制I/R后小胶质细胞的活化增殖。Objective:To observe the effects of high concentration of hydrogen on the hippocampal CA1 neurons and microglia in rats with global cerebral ischemia / reperfusion(I/R).Methods:75 male SD rats were randomly divided into 3 groups by using a random number table.Sham operation(SH) group 15 rats,using electrocoagulation of bilateral vertebral artery,only exposed the bilateral common carotid arteries but not clipped;Model(4vo.con) group 30 rats,with 4 vessel occlusion(4-VO) established global cerebral I/R injury model and reperfusion and inhalation of 67% N2,33% O_2;Hydrogen treatment(4vo + H_2) group with 30 rats,after established model,reperfusion and inhalation of 67% of H2,33% O2,after 3 d and 5 d treatment,morphology and number of hippocampal CA1 neuronal cell were observed and analyzed.Results:Neurons in hippocampal CA1 region of model group were arranged loosely with obvious degeneration,nuclear condensation,a large number of Nissl bodies lost.The CA1 neurons in the hippocampus of the hydrogen treatment group were slightly loose,the cell structure was relatively complete,the number of Nissl body cells decreased,and the pathological injury was significantly reduced compared with the model group.After hydrogen treated for 3 days or 5 days,the number of neurons was much more than that in the model group but lower than that of sham operation(SH) group,the differences were statistically significant(P〈0.05);The number of microglial cells in hydrogen treatment( 4vo + H2)group was more than that in the sham operation group,but lower than that of model(4vo.con) group,the differences were statistically significant(P〈0.05).Conclusion:Inhalation of high concentration hydrogen has excellent protective effect on the neurons of rat hippocampal CA1 region during reperfusion.The possible mechanism may be that hydrogen could inhibit the microglia activation and proliferation during global cerebral ischemia/reperfusion.
