摘  要:目的探讨胃泌素释放肽前体(ProGRP)和神经元特异性烯醇化酶(NSE)在小细胞肺癌(SCLC)诊断中的临床意义。方法应用化学发光法分别测定33例SCLC患者、37例NSCLC患者、30例肺部良性疾病患者及42例健康体检者的外周血ProGRP和NSE水平,观察其在诊断SCLC时的临床意义。结果 ProGRP和NSE在SCLC患者外周血中水平明显高于其他组,ProGRP和NSE诊断SCLC的灵敏度分别为63.6%和57.6%,两者联合检测的灵敏度明显升高,达到81.8%;ProGRP和NSE诊断SCLC的特异度分别为97.2%和98.2%,两者联合检测的特异度为96.3%。结论针对SCLC、ProGRP和NSE在单项检测时均有很好的特异度,但是灵敏度均不够理想,两者联合检测时特异度保持较高水平,同时可以获得更高的灵敏度。ProGRP和NSE都可以作为SCLC的外周血标志物,尤其是两者联合检测,在SCLC的临床诊断中发挥积极重要的作用。Objective To explore the clinical value of ProGRP and neuronspecific enolase( NSE) in the diagnosis of small cell lung cancer( SCLC). Methods The ProGRP and NSE levels were tested by electrochemiluminescence respectively in the peripheral blood samples of 33 cases with SCLC,37 cases with NSCLC,30 cases with benign disease and 42 health controls. Results The levels of ProGRP and NSE in SCLC patients were significantly higher than those in other groups. In the diagnosis of SCLC,the sensitivity of ProGRP and NSE was respectively 63. 6% and 57. 6%,and the sensitivity of combined test was significantly higher to 81. 8%; the specificity of ProGRP and NSE was respectively 97. 2% and 98. 2%,and the specificity of combined test was 96. 3%. Conclusion In the diagnosis of SCLC,the specificity of single test with ProGRP or NSE is high,but the sensitivity was not satisfying,so under the combined test of ProGRP and NSE,the specificity is higher,and higher sensitivity can be obtained. ProGRP and NSE can be used as peripheral blood markers,especially for the combined test,which play an important positive role in the clinical diagnosis of SCLC.
