以紫癜为首发表现的外周T细胞淋巴瘤1例

摘　　要：患者男,63岁。双下肢反复瘀斑、瘀点伴剧痒半年,加重3个月。查体:全身可见密集或散在的瘀斑、瘀点,部分融合成片,以双下肢为甚,腋窝可扪及两粒黄豆大小的肿大淋巴结,活动可。皮损组织病理示:局部血管周围可见红细胞渗出及含铁血红素沉着。淋巴结病理示非霍奇金淋巴瘤;免疫组织化学示:CD43(+),CD45RO(+)。诊断:外周T细胞淋巴瘤,非特殊类型。A 63-year-old male presented with repeated ecchymosis and petechiae accompanied by severe itching on clouble lower limbs for half of a year, aggravated 3 months latter. Physical examination found that ecchymo- sis and petechiae were distributed on his whole body scatteredly or densely,partially fused into pieces,es- pecially in both lower limbs. There were two bean-sized lymph nodes in axillary fossa, which is active by touching,however the other systems were normal. Histopathology of biopsy from the skin showed that there were exudation of RBCs and hemosiderin pigmentation around the local blood vessels. Lymph node pathol- ogy revealed Non-Hodgkin＇ s Lymphoma. CD43 （ ＋ ） and CIM5RO （ ＋ ） were shown by immunohisto- chemistry. Diagnosis of peripheral T-cell lymphoma- unspecified was established.
