外科引流冲洗在食管癌贲门癌术后并发症中的应用

摘　　要：目的探讨外科引流冲洗在食管癌、贲门癌术后并发症中的应用。方法回顾分析2010年1月至2014年12月收治的食管癌及贲门癌手术患者临床资料，对术后发生吻合口瘘26例行瘘腔引流冲洗、脓胸18例行胸腔引流冲洗、切口感染11例行切口引流冲洗，并观察各自疗效。结果26例吻合口瘘患者中24例瘘口愈合，11％因瘘口较大，在数字减影血管造影机（DSA）下置入支架后恢复良好，1例因继发感染性休克、多器官功能衰竭经治疗无效；对于18例脓胸患者以及11例切口感染患者，经外科引流冲洗后全部治愈。结论外科引流冲洗在食管癌、贲门癌术后并发的吻合口瘘、脓胸以及切口感染中起到了显著的疗效，在临床上值得推广。Objective To investigate the application of surgical drainage and irrigation in the postoperative complications of esophageal carcinoma and cardiac carcinoma. Methods Clinical data of 411 patients with esophageal cancer and cardiac cancer treated from January 2010 to December 2014 were analyzed retrospectively, 26 cases of anastomotic leakage after operation were washed with fistula cavity drainage, 18 cases of empyema with pleural drainage, 11 cases of incision infection with incision drainage and irrigation, and observe the respective curative effect. Results Among 26 cases of anastomotic fistula patients, 24 cases were cured, 1 case with large fistula recovered well after the stent was placed in the digital subtraction angiography （ DSA ） , 1 case with septic shock and MODS was ineffective after active treatment. For 18 cases with empyema and11 cases with incision infection patients, all were cured by surgical drainage and irrigation. Conclusion Surgical drainage and irrigation plays a significant effect in postoperative anastomotic fistula, empyema, and incision infection that the carcinoma of esophagus and gastric cardia supervene with, which is worth popularizing in clinical practice.
