摘  要:准确的骨龄评估可以评价个体发育所处的阶段,为正畸临床选择矫治时机和预测颅面生长发育潜力提供重要的参考依据。本文从骨龄评估颅面生长发育的研究现状、影响骨龄评估颅面生长发育准确性的因素、骨龄评估颅面生长发育的改进与发展趋势三个方面展开论述,观点总结如下。1手腕骨龄,尤其是Fishman手腕骨龄分期法是一种有机的、相对简单并可靠的骨龄评估方法;颈椎骨龄分期法不用接受额外的X线照射,易于观察下颌生长高峰期,尤其是适应当今影像数字化趋势的颈椎骨龄定量分期法;2骨龄准确性受种族、性别、评定方法本身特点等因素的影响,应找到适合特定患者的骨龄评估方法;3环境因素,尤其是生物钟长期紊乱可导致骨发育异常,影响骨龄评估的准确性,提出通过调节生物钟治疗颅面生长发育异常的新方法,并期望发现评价颅面生长发育的新指标、新标准,提高骨龄评估生长发育的准确性。Accurate skeletal age evaluation can assess the stage of individual development,and therefore provide the important reference for orthodontists to choose proper treatment timing and predict the growth potential. This paper explores three aspects,including the research status of the assessment of craniofacial growth and development,the influencing factors of skeletal age evaluation accuracy,and the improvement and development of skelatal age evaluation. The views are summarized as follows: 1The hand-wrist skeletal age,especially Fishman Skeletal Maturity Indicators( SMI),is organic,simple and reliable. The method of Cervical Vertebrae Maturation( CVM),especially Quantitative Cervical Vertebral Maturation( QCVM),has attracted more and more attention,and there is no need for additional x-ray exposure and it is easy to observe the mandibular growth peak. 2The accuracy of skeletal age evaluation is affected by several factors,such as race,gender and evaluation methods. Therefore,it's necessary to find the proper evaluation method for specific patients. 3Environmental factors can lead to abnormal bone development,especially the long-term disorders of biological clock,which influences the accuracy of skeletal age evaluation. The new methods is proposed,regulating the biological clock to modify the abnormal craniofacial development and improve the assessment accuracy with skeletal age.
