摘　　要：目的通过对围术期膀胱癌患者输血前后T淋巴细胞群及NK细胞活性的变化的观察,探讨输血对膀胱癌患者免疫功能的影响。方法随机选择70例膀胱癌手术病例,分成输血组(BT)47例与非输血组(NBT)23例,分别采集手术前1周之内,以及术后第1天、第3天、第7天的外周静脉血,用流式细胞仪测定T淋巴细胞亚群CD_3^+、CD_4^+、CD_8^+及NK细胞活性,并计算CD_4^+/CD_8^+比值,分析其变化规律。结果 BT组与NBT组术前各项指标相比较无显著性差异(P>0.05);术后第1天,BT组和NBT组各项指标相比较于术前均有显著性差异(P<0.05),但BT组与NBT组各项指标相比较无显著性差异(P>0.05);术后第3天、第7天,BT组各项指标相比较于术前有显著性差异(P<0.05);NBT组各项指标相比较于术前无显著性差异(P>0.05);BT组各项指标与NBT组相比较均有显著性差异(P<0.05)。结论输血对膀胱癌患者的细胞免疫功能有较强的抑制作用。Objective To study the influence of perioperative blood transfusion on immunologic function of bladder cancer sufferers through the observation on the changes of T lymphocyte mass and NK cell activity before and after blood transfusion.Methods 70 cases of bladder cancer surgery were selected randomly and divided into blood transfusion group（BT） of 47 cases and non-blood transfusion group（NBT） of 23 cases.Peripheral venous blood of the samples was obtained one week before the surgery, and on the 1st, 3rd and 7th day after surgery.CD_3~＋,CD_4~＋,CD_8~＋ of T lymphocyte subsets and the cell activities of NK were tested by flow cytometry（FCM）,and the ratio of CD_4~＋/CD_8~＋ was calculated, then the change regularities were analyzed. Results There was no significant difference in the indicators between BT group and NBT group before surgery（P〉0.05）; the indicators in both BT group and NBT group showed clear changes on the 1st day after surgery compared with those before surgery（ P〈0.05）, but the difference in indicators between BT group and NBT group was not obvious（ P〉0.05）; the indicators of BT group on the 3rd and 7th day after surgery had significant differences compared with the preoperative results（ P〈0.05）, while the indicators of NBT group changed little compared with the preoperative results（ P 〉0.05）.The indicators between BT group and NBT group showed significant differences（P〈0.05）.Conclusion Perioperative blood transfusion shows strong inhibitory effect on immunologic function of bladder cancer sufferers.
