摘 要:目的:探讨三阴性乳腺癌Sema4D与c-met表达及其与微淋巴管密度(LMVD)和各项临床病理特征的关系。方法:采用免疫组化方法检测95例三阴性乳腺癌组织中Sema4D与c-met表达,检测微淋巴管密度(LMVD),依据不同临床病理特征分组并进行组间比较,同时以78例非三阴性乳腺癌及22例正常乳腺组织标本做对照。结果:三阴性乳腺癌组织中,Sema4D与c-met阳性表达率均明显高于其在非三阴性乳腺癌及在正常乳腺组织中的阳性表达率;Sema4D与c-met的表达具有正相关性(r=0.713,P<0.05)。三阴性乳腺癌组织中LMVD明显高于非三阴性乳腺癌及正常乳腺组织(P<0.05);Sema4D、c-met阳性表达组的LMVD明显高于阴性表达组(P<0.05);Sema4D表达在肿瘤大小、淋巴结转移数目及是否远处转移间比较,差异有显著性意义(P<0.05)。c-met表达在淋巴结转移数目及是否远处转移间比较,差异有显著性意义(P<0.05)。结论:Sema4D与c-met的高表达可能促进三阴性乳腺癌淋巴管的新生、淋巴道转移及远处转移,相关抑制剂的研究有可能成为预防和治疗三阴性乳腺癌淋巴道转移和远处转移的新方法。Objective:To investigate the expression of Sema4 D and c-met in human triple negative breast cancer and their relationship with clinical pathological indicators.Methods:The expressions of Sema4 D and c-met in 95 cases of triple-negative breast cancer,78 cases of non-triple-negative breast cancer and 22 cases of normal breast tissue were detected by immunohistochemistry and the LMVD of all specimens was measured,and their relationship with lymphatic vessel density and clinicopathological parameters was analyzed.Result:The positive rates of Sema4 D and c-met were significantly higher than that in non-triple-negative breast cancer and in normal breast tissue(P〈0.05).Sema4 D expression was positively correlated with c-met(r=0.713,P〈0.05).The LMVD of triple-negative breast cancer was significantly higher than that of non-triple-negative breast cancer and normal breast tissue(P〈0.05).Higher LMVD was found in triple-negative breast cancer with Sema4 D or c-met positive expression than with negative expression(P〈0.05).In number of metastatic lymph nodes,distant metastasis and/or tumor size,TNBC with Sema4 D and c-met positive expression have significantly worse performance(P〈0.05).Conclusion:Over expression of Sema4 D and c-met in triple-negative breast cancer may promote lymphangiogenesis,lymphatic metastasis and distant metastasis.Correlated study of inhibitor may become the new therapy of lymphatic metastasis and distant metastasis of triple-negative breast cancer.
