胃癌患者血清中MiR-374a的表达及其对胃癌预后判断的价值

摘　　要：目的:探讨miR-374a表达水平与胃癌临床及预后的潜在相关性。方法:实时荧光定量PCR检测胃癌病人及来源于常规体检的健康人血清的miR-374a表达水平,然后对胃癌病人miR-374a表达水平和一系列临床参数(肿瘤大小、TNM分期、病理分期、总生存率、无复发生存率及预后等)进行分析。结果:实时荧光定量PCR结果显示胃癌病人血清miR-374a表达水平比正常健康人明显升高(P<0.01),而且与肿瘤大小、TNM分期、病理分期等临床病理学参数密切相关。miR-374a高表达的胃癌病人总生存率及无复发生存率均比miR-374a低表达的胃癌病人明显下降。多因素分析结果显示血清miR-374a表达水平可以用来作为判断胃癌预后的独立因素。结论:血清miR-374a表达水平在胃癌病人中明显升高并且和不良预后密切相关,因此血清miR-374a有可能成为预测胃癌预后的分子标志物。Objective： To find out the potential relationship between miR-374 a expression and gastric cancer prognosis. Methods： Real-time quantitative PCR was performed to evaluate the expression level of serum miR-374 a from gastric cancer patients and healthy controls. Then the correlation between miR-374 a expression and a variety of important clinical parameters（ such as tumor size,TNM size and tumor grade,overall survival,relapse free survival and prognostic value） was further studied. Results： The expression level of miR-374 a was significantly higher in the serum of gastric cancer patients than that of healthy controls（ P〈0. 01）. In addition,miR-374 a expression was associated with various gastric cancerclinicopathological parameters including tumor size,TNM size and tumor grade. The high serum miR-374 a expression group suffered both poorer relapse-free survival and overall survival. In the multivariate analysis model,serum miR-374 a was identified as an independent prognostic marker.Conclusion： Serum miR-374 a up-regulation is associated with poor prognosis in gastric cancer and it may be employed as a promising marker for predicting the clinical outcome of gastric cancer patients.
