摘 要:目的:探讨各期妊娠期及各年龄段母体25-(OH)D及甲状腺激素的水平,并了解二者间的相关性。方法:选取2014年8月至2015年7月既往无甲状腺相关疾病史的妊娠孕妇1 029名。所有孕妇按孕周分为孕早期组(0~12+6周)、孕中期组(13~27+6周)、孕晚期组(28周~分娩);按年龄分为A组(〈25岁)、B组(≥25岁而≤30岁)、C组(〉30岁)。运用串联质谱仪检测维生素D水平,甲状腺相关激素由全自动电化学发光免疫分析仪完成测定。结果:25-(OH)D和甲状腺激素水平在各年龄组间差异无统计学意义(P〉0.05),而25-(OH)D水平随着年龄升高呈升高的趋势。25-(OH)D水平随着孕期增加而升高,差异有统计学意义(P〈0.05),孕妇在不同妊娠期血清25-(OH)D营养缺乏情况普遍存在,组间比较差异有统计学意义(P〈0.05);FT3、FT4随着孕期增加而降低,而TSH水平孕中期及孕晚期明显升高,差异均有统计学意义(P〈0.05)。25-(OH)D与甲状腺激素间无相关性(P〉0.05)。结论:孕妇妊娠阶段与25-(OH)D及甲状腺激素水平密切相关,甲状腺激素水平与25-(OH)D无相关,孕妇普遍存在25-(OH)D不足或缺乏。Objective: To discuss the levels of 25-hydroxy vitamin D[25-( OH) D]and thyroid hormones at different gestational stages and different gestational ages,and assess the relationship between them. Methods:1 029 pregnant women without thyroid related medical history from August 2014 to July 2015 were selected for the study. All these gravidas were divided into first trimester group( 0-12^+ 6weeks),second trimester group( 13-27^+ 6weeks) and third trimester group( 28 weeks-delivery) by the duration of pregnant. 1 029 pregnant women were divided into group A( 〈25 years old),group B( ≥25 and ≤30 years old),group C(〉 30 years old) according to the age. The detection of 25-hydroxy vitamin D were performed using tandem mass spectrometer. Thyroid hormones were measured by electrochemical immunoassay. Results: The difference of 25-( OH) D and thyroid hormones were not found among the three age group( P〈0. 05),but 25-( OH) D increased with age. 25-( OH) D and TSH differently increased with pregnancy cycle( P〈0. 05),25-( OH) D nutrition deficiency was common in pregnant women at different gestational stages,and there were significant differences among the three groups( P〈0. 05).FT3 and FT4 differently decreased( P〈0. 05). 25-( OH) D level was not correlated with thyroid hormones( P〈0. 05). Conclusion: 25-( OH) D and thyroid hormones levels are closely related to the gestational stages,and there is not correlative between thyroid hormones and 25-( OH) D. Pregnant women widely have deficiency of 25-( OH) D.
