舒肝解郁胶囊治疗卒中后抑郁及对康复的影响

摘　　要：目的:观察舒肝解郁胶囊治疗脑卒中后抑郁症状及神经功能康复的作用。方法:将脑卒中后抑郁(轻中度)患者55例按随机数字表法分为治疗组(舒肝解郁组)28例和对照组(舍曲林组)27例,2组患者均给予常规药物治疗,同时进行肢体功能康复治疗及心理辅导。治疗组加服舒肝解郁胶囊720 mg bid,对照组加服舍曲林50 mg qd,疗程12周。治疗前后采用HAMD减分率评价临床疗效,同时采用中国卒中患者神经功能缺损评分标准(CSS)评价神经功能缺损情况,采用巴氏量表(Barthel指数)评价日常生活能力(ADL)情况,采用TESS副反应量表评价其安全性。结果:按照HAMD减分率临床疗效标准,治疗组临床控制4例占14.3%,显著进步11例占39.2%,进步9例占32.1%,无效4例占14.2%,总有效率为85.7%。对照组临床控制5例占18.5%,显著进步9例占33.3%,进步10例占37.0%,无效3例占11.1%,总有效率为88.9%,2组总有效率比较差异无统计学意义。治疗后治疗组与对照组CSS评分均明显下降,Barthel指数明显升高,组内治疗前后比较差异有统计学意义,组间疗效对照差异无统计学意义。结论:舒肝解郁胶囊治疗卒中后抑郁与舍曲林疗效相当,可明显提高康复疗效,且不良反应明显减少,表明舒肝解郁胶囊治疗卒中后轻中度抑郁的有效性和安全性。Objective： to observe the effects of Shuganjieyu capsule in treating the depressive symptoms after stroke and the recovery of neural function. Methods： 55 patients with depression（ mild and moderate） after stroke were randomly divided into treatment group（ Shuganjieyu capsule group）,28 cases and the control group（ sertraline group）,27 cases.Two groups of patients were given routine drug treatment,as well as limb function rehabilitation and psychological counseling. The treatment group took 720 mg bid of Shuganjieyu capsule,and the control group took 50 mg qd of sertraline for 12 weeks. Clinical curative effects are evaluated by HAMD deduction rate before and after the treatment; at the same time nerve function defect is evaluated by Chinese scale of neurological deficit of stroke patients（ China Stroke Scale,CSS）; ability of daily life（ ADL） is assessed by Barthel index and its safety is assessed by TESS scale. Results：According to the clinical efficacy of HAMD deducted rate standard,the results of treatment group are as follows： 8 cases controlled（ 28. 6%）,8 cases significantly improved（ 28. 6%）,9 cases improved（ 32. 1%）,3 cases invalid（ 10. 7%）— the total effective rate was 89. 3%. As for the control group,the results are as follows： 8 cases controlled（ 29. 6%）,9cases significantly improved（ 33. 3%）,7cases improved（ 25. 9%）,3 cases invalid（ 11. 1%） — the total effective rate was 88. 9%. There is no significant difference as for the total effective rate. After treatment both in the treatment group and the control group,CSS scores significantly dropped,and Barthel index increased significantly. Inside both groups,the curative effect is significantly different after treatment; but between the two groups,the curative effect is of little difference.Conclusion： Shuganjieyu capsule has the same efficacy as sertraline in the treatment of post-stroke depression.
