小鼠口腔癌淋巴道转移模型颌下淋巴结播散肿瘤细胞的检测

摘　　要：目的:检测小鼠口腔癌淋巴道转移模型癌变过程不同时期的颌下淋巴结播散肿瘤细胞(disseminated tumor cell,DTC),探讨小鼠口腔癌变过程与颌下淋巴结DTC的关系。方法:病理检查明确诊断为口腔正常组小鼠4只,轻至中度异常增生组8只、重度异常增生组4只,口腔鳞癌无淋巴结转移组、伴淋巴结转移组各4只。采用免疫组织化学指标广谱细胞角蛋白(pancytokeratin,pan-CK)检测颌下淋巴结DTC,比较其差异。结果:口腔正常组0只、轻至中度异常增生组2只,重度异常增生组1只,口腔鳞癌无淋巴结转移组2只,口腔鳞癌伴淋巴结转移组4只颌下淋巴结发现DTC,阳性率分别为0%(0/4)、25%(2/8)、25%(1/4)、50%(2/4)和100%(4/4);随着癌病程度的增加,DTC由簇状分布向片状分布发展。结论:小鼠口腔黏膜恶变过程中,区域淋巴结在轻至中度异常增生时即可检测到DTC,并随癌变程度加重,DTC的发生率及数量不断增加。Objective：To detect disseminated tumor cells（DTC）in submandibular-lymph node tissues of oral carcinoma mouse model and to investigate the relationship between carcinogenesis of the oral mucosa and DTC at different stages.Methods：The submandibular lymph node tissues were collected from 4normal control mice,8mice with mild-to-moderate dysplasia,4mice with severe abnormal hyperplasia,4mice with squamous cell carcinoma（OSCC）without lymph node metastasis and 4mice with OSCC combined lymph node metastasis.DTC was detected by immunohistochemistry with epithelial-specific pan-cytokeratin antibodies.Results：In submandibular lymph node tissue DTCs were detected in 0of 4（0%）cases in normal mucosa group,2of 8（25%）cases in mild-to-moderate dysplasia group,1of 4（25%）cases of severe abnormal hyperplasia group,2of 4（50%）cases of OSCC group without lymph node metastasis and 4of 4（100%）cases of OSCC group with lymph node metastasis.As the pathological degree of oral carcinoma increases,the distribution of DTCs were found from scattered single cell to aggregated cell masses.Conclusion：Primary oral cancer cells may disseminate to submandibular lymph node tissues in the stage of mild-to-moderate dysplasia.The rate and number of DTC increased gradually from the stage of mild-to-moderate dysplasia to the stage of OSCC with lymph node metastasis.
