摘 要:目的 :探讨PPM1D m RNA表达与肝癌预后的相关性。方法:提取86例肝癌患者癌组织及癌旁肝组织总RNA,q PCR法检测PPM1D m RNA表达量,免疫组化检测蛋白表达水平。根据癌旁肝组织中PPM1D m RNA表达量,将肝癌患者分组为高表达组与低表达组,对两组患者临床资料及生存时间进行统计分析。结果:PPM1D m RNA在肝癌组织中表达水平显著高于癌旁组织,免疫组化检测蛋白表达水平证实上述结果。以癌旁肝组织PPM1D m RNA表达量为阈值,高表达组56例,低表达组30例。两组患者的AFP水平、肿瘤大小、肿瘤TNM分期以及肿瘤复发、家族史等临床病理因素差异有统计学意义(P<0.01);年龄、性别、门静脉侵犯、淋巴结转移、HBV感染及酒精摄入史等因素差异无统计学意义(P>0.05)。高表达组患者中位生存期为13个月,低表达组为32个月。结论:PPM1D m RNA表达水平可能与肝癌恶性程度相关,可能成为肝癌预后的预测因子。Objective: Protein phosphatase magnesium- dependent 1δ(PPM1D)is an oncogene, overexpressed in many solid tumors, including ovarian cancer and breast cancer. The current study examined the expression and the prognostic value of PPM1 D m RNA in human hepatocellular carcinoma(HCC). Methods: Total RNA was extracted from 86 HCC and paired non- cancerous liver tissues. PPM1 D m RNA expression was determined by real- time quantitative reverse transcrip-tase- polymerase chain reaction(q PCR). Immunohistochemistry assay was used to verify the expression of ppm1 d protein in the HCC and non- cancerous liver tissues. HCC patients were grouped according to PPM1 D m RNA expression with the average PPM1 D m RNA level in non- cancerous liver tissue samples as the cut- off. Correlations between clinicopathologic variables, overall survival and PPM1 D m RNA expression were analyzed. Results: PPM1 D m RNA was signi cantly higher in HCC than in the paired non- cancerous tissue(P〈0.01). This was confirmed by ppm1 d staining. 56 patients were classified as high expression group and the other 30 patients were categorized as low expression group. There were significant differences between the two groups in term of alpha- fetoprotein(α- FP)level(P〈0.01), tumor size(P〈0.01), TNM stage(P〈0.01), recurrence incidence(P〈0.01)and family history of liver cancer(P〈0.01). The current study failed to find significant differences between the two groups in the following clinical characteristics: age, gender, portal vein invasion, lymphnode metastasis, hepatitis B virus(HBV)infection and alcohol intake. Survival time of high expression group was significantly shorter than that of low expression group(median survival, 13 months and 32 months,respectively, P〈0.01). Conclusion: Up- regulation of PPM1 D m RNA was associated with progressive pathological feature and poor prognosis in HCCpatients. PPM1 D m RNA may serve as a prognostic marker in HCC.
