综合性医院床位配置的探讨

摘　　要：目的探讨一种科学的、有效的、可操作性强的综合性医院病床分配方法,为综合性医院调整科室床位配置提供决策依据。方法提取某综合性医院2004年-2014年的统计数据,利用黄金分割法,按照统计学原理建立病床分配模型,计算各科室病床95%预测置信区间。结果病床预测区间结果表明,全院18个临床科室中,有55.56%科室需要调整,其中有6个科室需增加床位,有4个科室需减少床位,其余44.44%科室病床设置合理,说明医院超过半数以上的科室床位配置存在不合理现象。根据结果全院共需减少床位27张,增加病床20张,总病床数减少7张,如果全院病床总数不变的情况下,也可以在部分临床科室间进行微调。结论导致综合性医院床位的紧张并非完全因为床位配备不够,而是因为床位的管理体制不够完善。综合性医院可以采用跨科收治患者、建立综合病区、实行床位弹性管理的方法有效缓解综合性医院用床紧张的现象。Objectives To investigate a scientific and effective, feasible, the hospital beds allocation methods for hospital beds, adjust department provides decision-making basis for configuration. Methods To extract statistical data from a comprehensive hospital from 2004 to 2014, establish beds assignment model according to statistics theory with the application of gold ratio method, and calculated the 95% confidence interval of each department.Results The results showed that the prediction interval beds, 18 departments in the hospital bed set reasonable with 8 departments, 10 departments needed to be adjusted, including 6 departments needed to increase beds and 4 departments needed to decrease beds, and other 44.44% departments beds assignment were reasonable, which indicated that bed configuration hospital department unreasonable phenomenon. According to the results, the hospital needed to decrease 27 beds and increase 20 beds, the total numbers should decrease 7 beds. If the total number of beds the hospital remained constant, we could also make fine-tuning among some of the departments.Conclusions The reason that caused the stressed state of hospital beds was not due to lacking of beds, but because of the imperfect management system of beds. The comprehensive hospital should adopt methods such as admitting patients across departments, establish comprehensive inpatients areas, implement flexible management method,thus to alleviate the intense situation of bed utilization in a comprehensive hospital.
