摘　　要：目的　研究153 　钐 -乙二胺四甲撑磷酸 (153 　Sm EDTMP)与化疗药物联合治疗肺癌骨转移的临床效果。方法　 110例肺癌已有骨转移而未作手术的患者接受了该治疗 ,其中男 82例 ,女 2 8例 ,年龄 3 2～76岁 ;原发癌分别为肺鳞癌 2 8例 ,肺腺癌 2 7例 ,小细胞肺癌 7例 ,混合型 41例 ,肺泡癌 7例。 110例中 ,单独用153 　Sm治疗 3 7例 ,153 Sm治疗后 3天加化疗 42例 ,化疗完成后 3 0天再加153 Sm治疗 3 1例。153 Sm EDTMP采用两次给药法 ,其剂量的范围为 1111～ 2 660MBq。小细胞肺癌用CCNU、MTX、CTX化疗方案 ;非小细胞肺癌用MMC、VCR、DDP方案。资料用卡方检验作统计分析。结果　单独153 Sm治疗者的总止痛有效率为89 .2 % ,153 Sm加化疗为 92 .8% ,化疗后加153 Sm为 90 .3 %。单独153 Sm治疗者病灶消失 9例 ,153 Sm加化疗消失 12例 ,化疗后加153 Sm消失 9例。单独153 Sm治疗者 1年生存率为 2 9.7% (11/ 3 7) ,153 Sm加化疗为 40 .5 %(17/ 42 ) ,化疗后加153 Sm为 3 8.7% (12 / 3 1)。结论　用153 Sm EDTMP联合化疗治疗肺癌骨转移有较好的临床效果。Objective To study the clinical effects of 153 Sm EDTMP plus chemotherapy in the treatment of bone metastasis of lung cancer. Methods One hundred and ten lung cancer patients with one metastasis [male 82 and female 28, aged from 32 to 76 yrs; squamous cell carcinoma 28, adenocarcinoma 27, small cell lung cancer (SCLC) 7, mix type 41, alveolar carcinoma 7] who did not undergo an operation were entered into this study. The patients were divided into 3 groups: 153 Sm EDTMP therapy only (37 cases), 153 Sm EDTMP plus chemotherapy after 3 days (42 cases), 30 days after chemotherapy plus 153 Sm EDTMP (31 cases). The dosages of 153 Sm EDTMP ranged from 1 111 to 2 660 MBq. The patients with SCLC were adapted CCNU, MTX and CTX; those with non small cell lung cancer (NSCLC) were adapted MMC, VCR and DDP. Statistic analysis of the data was performed by χ 2 test. Results Total pain relief rate for 153 Sm EDTMP only was 89.2% , for 153 Sm EDTMP plus chemotherapy was 92.8%, and for chemotherapy plus 153 Sm EDTMP was 90.3% . The foci disappeared in 9 cases with 153 Sm EDTMP only, in 12 cases with 153 Sm EDTMP plus chemotherapy, and in 9 cases with chemotherapy plus 153 Sm EDTMP. The 1 year survival rate was 29.7%(11/37) by 153 Sm only, 40.5%(17/42) by 153 Sm plus chemotherapy, 38.7%(12/31) by chemotherapy plus 153 Sm EDTMP. Conclusion 153 Sm EDTMP plus chemotherapy is effective in the treatment of bone metastasis of lung cancer.
