摘　　要：目的　探讨螺旋CT多平面重建 (SCTMPR)在儿童脊髓栓系综合征 (TCS)诊断中的应用。资料与方法　对 118例TCS患儿行SCT平扫 ,然后进行MPR。结果　 118例TCS患儿中脊髓低位 111例 ,脊髓位置正常 7例。低位脊髓合并脊髓纵裂者 12例 ,椎管内脂肪瘤 2 4例 ,椎管内皮样囊肿 9例 ,脊髓空洞症 9例 ,半椎体及蝴蝶椎 12例 ,棘突缺如或分叉 16例。脊髓位置正常合并脊髓空洞症 2例 ,脊髓纵裂 3例。 118例TCS患儿中 112例伴有脊柱裂(占 94.9%)。其中包括 14例隐性脊柱裂及 98例显性脊柱裂。通过MPR均能获得较满意的图像 ,满足诊断的需要。结论　MPR可以较全面、立体、直观地显示TCS患者脊髓及脊椎的情况 ,是继MRI之后诊断TCS的又一重要方法。Objective To evaluate spiral CT multiplanar reformation (SCT MPR) in the diagnosis of tethered cord syndrome (TCS) in children. Materials and Methods Plain spiral CT scanning was performed in 118 children with TCS. SCT MPR images were obtained by using special software on a workstation. The imaging findings were analyzed.Results The spinal cord was terminated at normal level in 7 cases, and below L 1～L 2 level in 111 cases,. Of 111 cases with lowly positioned spinal cord, diastematomyelia was accompanied in 12, intraspinal canal lipoma in 24, intraspinal canal dermoidcyst in 9, syringomyelia in 9, hemivertebra or cleft vertebra in 12, and spinal process deformities in 16. Of 7 cases with normally positioned spinal cord, syringomyelia was accompanied in 2 cases and diastematomyelia in 3 cases. Of the total 118 cases of TCS, spinal bifida was accompanied in 112 (94.9%), of which 14 cases were occult spinal dysraphism and 98 cases were secondary dysraphism. Satisfactory high quality MPR images, enough to meet the diagnostic requirements, were obtained in all cases.Conclusion SCT MPR can stereoscopically and directly display the total spinal cord and the vertebral structures, therefore, it is a useful technique in the diagnosis of TCS.
