摘　　要：目的分析2011~2014年间山东省某三甲医院宫颈癌患者住院费用的变化情况及其影响因素,为制定合理的医疗费用控制措施提供依据。方法收集住院患者的病案信息资料,通过描述性分析方法分析患者的基本情况和住院费用情况,采用单因素方差分析和逐步回归分析探索影响宫颈癌住院费用的主要因素。结果总体上宫颈癌患者的住院费用比较平稳,但平均住院天数总体呈下降趋势,从2011年的15.55d降至2014年的13.50d。宫颈癌患者平均住院费用中,比例最大的是药品费（8 298.44元占41.64%）,其次是卫生材料费（2 907.96元占14.59%）、手术费（2 495.78元占12.52%）和治疗费（2 479.91元占12.44%）;药占比从2011年的43.36%降至2014年的39.53%。多因素分析显示,住院天数是影响住院费用的主要因素（P〈0.05）。结论应通过进一步缩短平均住院天数、减少药品费用以及进一步完善医疗保障制度来控制医疗费用的过快增长。Objective To analyze the change trend and its influencing factors of hospitalization expenditure of cervical cancer patients from 2011 to 2014in a tertiary hospital of Shandong province,so as to provide basis for working out reasonable control measures for medical expenditure. Methods The information data on the first page of the medical record for cervical cancer patients were collected.Descriptive analysis,one-way ANOVA and stepwise analytical regression were used. Results The overall hospitalization expenditure of cervical cancer patients was stable.The average hospitalization days were decreasing in general and down from 15.55 days in 2011 to 13.50 days in 2014.The largest proportion of hospitalization expenditure was medicine expense（$8 298.44,41.64%）.The medical materials expenses,operation fees and drug fees were 14.59%（$2 907.96）,12.52%（$2 495.78）and 12.44%（$2 907.96）respectively.The proportion of drugs out of hospitalization expenditure decreased from 43.36%in 2011 to 39.53%in 2014.Multi-factor analysis showed that the hospitalization days were the main influencing factor on hospitalization expenditure. Conclusion The average hospitalization days should be shortened,drug costs reduced to improve the medical insurance system,so as to control the rapid growth of medical expenditure.
