单纯性肥胖儿童血中瘦素水平及其调控因子研究

摘　　要：目的　研究单纯性肥胖儿童血中瘦素水平及其调控因子。方法　检测 42对 1 0～ 1 2岁肥胖儿童血中瘦素 (Leptin)及其调控因子胰岛素 (Ins)、神经肽Y(NPY)和肿瘤坏死因子α(TNFα)。结果　肥胖儿童血中Lepin含量 [(9.33± 2 .1 4 )ng ml]非常显著地高于对照组含量 [(3 .47± 2 .57)ng ml] (P <0 .0 1 ) ,肥胖组Ins[(2 4 .75± 1 2 .75)mIu L]亦高于对照组 [(1 9.1 9± 6 .39)mIu L] (P <0 .0 5) ,肥胖组TNFα[(1 2 .62± 4 .50 )ng ml]非常显著地低于对照组 [(1 4 .43± 5 .56)ng ml] (P <0 .0 1 ) ,NPY两组间差异无显著性。女性血中Leptin水平 [(1 0 .0 0± 1 .1 2 )ng ml]显著高于男生 [(3 .72± 1 .1 1 )ng ml] (P <0 .0 1 ) ,重度肥胖组Leptin水平 (1 6 .59ng ml)高于轻度肥胖组 (7.91ng ml) (P <0 .0 5)。结论　肥胖儿童血中Leptin含量升高 ;肥胖者体内Ins含量增加 。Objective To study leptin level in blood of simple obese children and its regulated factors.Methods The contents of leptin, insulin, neuropeptide Y and tumor necrosis factor α were measured in blood of 42 pair simple obese children aged 10～12 years. Results The obese group had higher blood leptin level (9.33±2.14 ng/ml) than control group (3.47±2.57 ng/ml) ( P <0.0l ). The obese children had a significant increase in insulin level(the obese group:24.75±12.75 mIu/L, the control group:19.19±6.39 mIu/L) ( P <0.05), and a significant decrease in tumor necrosis factorαlevel(the obese group:12.62±4.50ng/ml, the control group:14.43±5.56ng/ml) ( P <0.01) as compared with their counterparts. The difference in neuropeptide Y between the obese and control groups was no significant. The leptin level in blood of girls (10.00±1.12ng/ml ) was significantly higher than that of boys( 3.72 ±1.11ng/ml) ( P <0 01). The leptin level in the severe obese group (16.59ng/ml) was higher than that in the mild obese group (7.91ng/ml) ( P <0.05).Conclusions The leptin content in blood of obese children has increased. The increase in insulin content and the decreases in the contents of neuropeptide Y and tumor necrosis factor α are related to the regulation and control of leptin.
