自身免疫性胰腺炎的MSCT表现分析

摘　　要：目的探讨MSCT在自身免疫性胰腺炎诊断中的应用价值。方法回顾性分析13例经临床确诊的自身免疫性胰腺炎患者的CT资料。所有患者均行CT平扫及动态增强扫描,10例患者行二维或三维重建。结果 13例患者中,8例表现为胰腺弥漫性肿大,5例表现为局限性肿大;8例胰腺周围可见有增厚的"包膜样"结构;7例可见胆总管胰腺段管壁不规则增厚,管腔狭窄;3例可见胰管扩张;9例可见脾血管受累;8例可见腹膜后淋巴结肿大;4例可见胰周脂肪间隙受累。结论自身免疫性胰腺炎具有特征性的CT表现,MSCT在自身免疫性胰腺炎的诊治中具有重要的应用价值。Objective To investigate the value of multislice CT（ MSCT） in the diagnosis of autoimmune pancreatitis（ AIP）.Methods CT data of 13 case with AIP who were confirmed by clinical therapy and / or histology,were retrospectively analyzed.They all underwent plain CT scanning and dynamic contrast-enhanced scanning. 2-D or 3-D reconstruction was performed in 10 cases. Results 8 cases showed diffuse pancreatic enlargement and 5 cases showed focal pancreatic enlargement. Capsule-like enhanced rim was found around the enlarged pancreas in 8 cases,Lumen stenosis of the distal common bile were found in 7 cases.Dilations of pancreatic duct was found in 3 cases. Splenic vessels were involved in 9 cases. 8 cases showed that post-peritoneum enlarged lymph nodes and 4 cases showed peripancreatic fat space was involved. Conclusion AIP showed some characteristics CT imaging findings,and MSCT plays an important role in the diagnosis of AIP.
