慢性B淋巴细胞白血病免疫表型的诊断研究

摘　　要：目的探讨慢性B淋巴细胞白血病(B-CLL)免疫表型的诊断情况。方法选择2014年1月至2015年2月潜江市江汉油田总医院初诊的50例B-CLL患者为观察组,再选择同期50例急性B淋巴细胞白血病(B-ALL)患者作为对照组。分别测定两组患者免疫细胞分型,并对比两组患者T细胞和自然杀伤细胞等免疫表型情况。结果 50例观察组患者中CD5、CD19以及CD20的表达较为常见,其阳性率分别为88.0%(44/50)、98.0%(49/50)、94.0%(47/50),CD22的表达阳性率较低,为78.0%(39/50),CD38最低,为36.0%(18/50)。观察组CD+3CD+4、CD+3CD+8、CD+4CD+8、CD+3、CD3-CD16+CD56+均高于对照组[(4.35±0.23)%比(3.21±0.45)%,(4.15±0.22)%比(3.02±0.32)%,(1.51±0.23)%比(1.10±0.11)%,(9.53±1.95)%比(6.46±1.54)%,(6.12±0.64)%比(4.35±0.12)%],差异有统计学意义(P<0.05或P<0.01)。结论 B-CLL的表型特征是CD19、CD23以及CD5的共同表达;CD10,CD5可以作为成熟型B-ALL和B-CLL鉴别诊断时特异性的指标。Objective To discuss the diagnosis of the immune phenotype of chronic lymphocytic leukemia B（ B-CLL）. Methods Total of 50 cases of B-CLL in Jianghan Olfield General Hospital from Jan. 2014 to Feb. 2015 were included as an observation group,and 50 cases of acute lymphoblastic leukemia B（ B-ALL） during the same period were included as a control group. They were measured for immune cell type,and T cells and NK cells immune phenotypes of the two groups were compared. Results In the observation group,the CD5,CD19 and CD20expression was more common,with the positive rate of 88. 0%（ 44 /50）,98. 0%（ 49 /50）,94. 0%（ 47 /50） respectively,while the positive rate of CD22 expression was relatively low,which was 78. 0%（ 39 /50） respectively,CD38 was the lowest,which was 36. 0%（ 18 /50） respectively.CD3＋CD4＋,CD3＋CD8＋,CD4＋CD8＋,CD3＋,CD3-CD16＋CD56＋of the observation group were all higher than the control group [（ 4. 35 ± 0. 23） % vs（ 3. 21 ± 0. 45） %,（ 4. 15 ± 0. 22） % vs（ 3. 02 ± 0. 32） %,（ 1. 51 ± 0. 23） % vs（ 1. 10 ± 0. 11） %,（ 9. 53 ± 1. 95） % vs（ 6. 46 ± 1. 54） %,（ 6. 12 ± 0. 64） % vs（ 4. 35 ± 0. 12） %,P〈0. 05 or P〈0. 01]. Conclusion Phenotypic characteristics of B-CLL is common expression of CD19,CD23 and CD5; CD10,CD5 can be used as specific diagnostic indicator for mature B-ALL and B-CLL identification.
