高危型HPV感染与宫颈癌前病变与宫颈癌的相关性研究

摘　　要：目的探讨高危型人乳头状瘤病毒(HPV)感染与宫颈癌前病变及宫颈癌的相关性,为临床治疗宫颈癌前病变和宫颈癌提供依据。方法选择2014年6月-2015年12月于医院进行治疗的244例患者,将其按宫颈细胞等改变分为癌前病变组189例及宫颈癌组55例,另选取同期健康体检者97名为对照组,对所有受试者采用宫颈刮片和宫颈液基细胞学检查法进行遴选筛查,采用达安PCR-反向点杂交法检测16种高危型HPV及3种低危型HPV,对各组受试者的检测结果进行对比并分析。结果 HPV阳性率癌前病变组为60.32%、宫颈癌组为85.45%、对照组为15.46%,3组受试者的HPV阳性率比较,差异有统计学意义(P<0.05);176例HPV感染患者中,检测为高危型HPV16型的患者有82例,占46.59%。结论高危型HPV感染在宫颈癌、宫颈癌前病变中占比较大,与两者之间存在着紧密的关系,并以HPV16型为主。OBJECTIVE To investigate the correlation between high-risk human papillomavirus（HPV）infection and cervical precancerous lesions and cervical cancer,and to provide the basis for the clinical treatment of cervical precancerous lesions and cervical cancer.METHODS Selected Totally 244 cases of female patients treated in the hospital for of from Jun.2014- Dec.2015 were selected in the study.The patients were divided into 189 cases of patients in precancerous lesion group,and 55 cases of cervical cancer group according to cervical cancer lesions.At the same time,97 healthy volunteers were selected as control group.For all subjects,Pap smear and liquidbased cervical cytology screening method was used for selection,and Encore PCR-reverse dot blot method was used to detect 16 kinds of high-risk HPV,and three kinds of low-risk HPV,the test results of patients in each group were compared and analyzed.RESULTS HPV positive rate of precancerous lesion group was 60.32%,HPVpositive rate of cervical cancer group was 85.45%,and HPV-positive rate of control group was 15.46%.The HPV infection rates of three groups of subjects were different,difference was significant（P0.05）.Among 176 cases of HPV-infected patients,there were 82 cases of high-risk HPV16,accounting for 46.59%.CONCLUSION High-risk HPV infection takes a large proportion in cancerous and precancerous cervical disease,and HPV16 is the main type.
