摘  要:目的探讨促红细胞生成素(EPO)对大鼠颈动脉球囊损伤后血管重构的影响。方法将75只大鼠随机分为对照组(假手术组)、阳性组(球囊损伤组)、EPO组、转化生长因子β1(TGF-β1)组、基质金属蛋白酶2(MMP-2)组,每组15只。阳性组采用PTCA球囊导管损伤颈动脉内膜制作动物模型,EPO组在阳性组基础上给于EPO3000U·kg^(-1)腹腔注射干预,TGF-β1组在EPO组基础上于颈动脉外膜涂抹质量浓度为0.5mg·mL^(-1) TGF-β1的普朗尼克凝胶,MMP-2组在EPO组基础上于颈动脉外膜涂抹质量浓度为0.5mg·mL^(-1) MMP-2的普朗尼克凝胶。分别于术后第1、7、14天检测新生血管内膜厚度、血管内膜/中膜面积比值、胶原表达及TGF-β1和MMP-2的表达,同时检测血清中血管内皮生长因子(VEGF)、肿瘤坏死因子α(TNF-α)水平。结果阳性组新生血管内膜厚度、血管内膜/中膜面积比值、胶原表达及TGF-β1和MMP-2的表达均较对照组明显增加,而EPO组以上各指标均较阳性组明显降低,但TGF-β1和MMP-2能逆转EPO的上述效果(均P<0.05)。结论 EPO能够抑制大鼠球囊损伤后血管新生内膜增生及抑制胶原表达,最终达到抑制血管再狭窄的效果,其作用可能与抑制TGF-β1、MMP-2、VEGF及TNF-α等炎症因子表达有关。Objective To explore the effect of erythropoietin(EPO)on vascular remodeling after carotid artery balloon injury.Methods Seventy-five SD rats were randomly divided into five groups,with 15 rats in each group.The sham operation was performed in control group.The PTCA balloon catheter was used to establish the animal model as the positive group via injuring carotid artery intima.Then,model rats were treated with intraperitoneal injection of 3000U·kg^-1EPO alone or in combination with smearing pluronic gel containing 0.5mg·mL^-1transforming growth factor beta1(TGF-β1)or 0.5mg·mL^-1 matrix metalloproteinase-2(MMP-2)on carotid adventitia.The neointimal thickness,intima/media area ratio,expression of collagen,TGF-β1and MMP-2,and serum levels of vascular endothelial growth factor(VEGF)and tumor necrosis factor-α(TNF-α)were detected on days 1,7and 14 after operation.Results Compared with control group,the neointimal thickness,intima/media area ratio,and expression of collagen,TGF-β1and MMP-2significantly increased in positive group.Compared with positive group,these parameters significantly decreased in EPO group.However,the effect of EPO was reversed by TGF-β1and MMP-2.Conclusion EPO can inhibit vascular restenosis following carotid artery balloon injury in rats through attenuating intimal hyperplasia and collagen expression.The weakened expression of inflammatory factors,such as TGF-β1,MMP-2,VEGF and TNF-α,maybe one of the important mechanisms of EPO action.
