摘　　要："互联网+"为创新社区健康管理服务提供了新的思路,针对目前社区健康管理存在的问题,提出利用移动互联网、物联网、智能传感技术、云计算技术、大数据技术等现代信息化技术手段,建立以社区健康服务团队为基础,以社区健康管理服务系统为依托,同时与区域人口健康信息平台网络化联接的互联网+社区健康管理服务模式,面向社区居民提供个性化、专业化的智慧健康管理服务。＂Internet plus＂ provides a new way for the innovation of community health management services, aiming at the existing problems of community health management, this paper proposes to use a series of modern information technology means such as mobile Internet, Internet of things, smart sensor technology, cloud computing technology, big data technology to establish the internet plus community health management services model which based on community health service team, relied on community health management service system and connected with the regional population health information platform, so as to provide personalized and professional intelligent health management services for community residents.
