根治性放疗联合外科挽救手术对扁桃体癌晚期预后的影响

摘　　要：目的：探讨根治性放疗＋外科挽救手术对扁桃体癌晚期患者预后的影响。方法选取2005年1月至2008年12月50例晚期扁桃体癌患者为研究对象，随机将其分为两组，对照组采取手术＋放疗治疗，观察组行根治性放疗＋外科挽救手术治疗，比较两组患者1、3、5年生存率及术后生活质量。结果观察组1、3、5生存率分别为84.0％、64.0％、40.0％，与对照组的88.0％、68.0％、44.0％比较差异未见统计学意义（P ＞0.05）。两组咀嚼评分[（78.6±25.4）分 vs（62.1±21.8）分]、交流评分[（92.0±18.2）分 vs（92.0±18.2）分]及总分[（696.1±90.8）分 vs（631.4±96.8）分]比较差异有统计学意义（P ＜0.05）。结论根治性放疗联合外科挽救手术与手术联合放疗综合治疗晚期扁桃体癌5年生存率类似，但前者相对能明显提高患者术后生活质量。Objective To investigate the effect of radical radiotherapy and surgical salvage oper-ation on the prognosis of patients with advanced tonsillar carcinoma. Methods Fifty cases of advanced tonsillar cancer patients were from January 2005 to December 2008 as the research objects,they were randomly divided into two groups,the control group was taken surgery and radiotherapy treatment,and the observation group was given treatment of radical radiotherapy and salvage surgery. The survival rate and life quality after operation of patients in the two groups for 1 year,3 years,5 years were compared. Results In observation group,the survival rates of 1 year,3 years and 5 years were 84% ,64% ,40% , which had no significant differences compared with control group of 88%,68% and 44%（P 〉 0. 05）. In addition,the two groups masticatory score[（78. 6 ±25. 4）vs（62. 1 ±21. 8）],AC score[（92. 0 ±18. 2） vs（92. 0 ±18. 2）]and total scores[（696. 1 ±90. 8）vs（631. 4 ±96. 8）],points between the two groups had significant differences（P 〈 0. 05）. Conclusions The treatment of advanced tonsillar carcinoma 5 years survival rate of radical radiotherapy combined with surgical salvage surgery compared to radiotherapy and surgery is similar,but the former relative can significantly improve the postoperative quality of life.
