经尿道双极等离子电切术治疗前列腺癌的临床疗效及生活质量评价

摘　　要：目的探讨经尿道双极等离子电切术治疗前列腺癌的临床疗效及生活质量。方法选取2009年7月至2015年7月间收治的80例前列腺癌患者,按照手术类型分为观察组和对照组,每组40例。观察组患者采用经尿道等离子电切术,对照组患者采用经尿道前列腺电切术,对比两组患者国际前列腺癌症状评分(IPSS)、最大尿流率、残余尿量、血清前列腺特异抗原(PSA)、尿管拔除时间、手术时间、术中出血量和生活质量。结果与治疗前比较,两组患者治疗后IPSS评分降低,最大尿流率增高,残余尿量减少,血清PSA水平降低,差异均有统计学意义(均P<0.05)。两组患者治疗后IPSS评分、最大尿流率、残余尿量和血清PSA水平的差异均无统计学意义(均P>0.05)。与对照组比较,观察组患者尿管拔出时间缩短,术中出血量较少,差异均有统计学意义(均P<0.05)。两组患者的手术时间比较,差异无统计学意义(P>0.05)。两组患者的躯体功能、角色功能、认知功能、情绪功能和社区功能评分差异均无统计学意义(均P>0.05)。结论经尿道双极等离子电切术治疗前列腺癌的临床疗效显著,与经尿道前列腺电切术无明显差异,但缩短了尿管拔出时间,并减少了术中出血量,可有效地缓解因前列腺癌造成的尿道梗阻症状,改善患者的生活质量。Objective To explore the clinical efficacy and quality of life of treatment for prostate cancer by transurethral plasmakinetic prostatectomy. Methods From July 2009 to July 2012 in People＇s Hospital of Linqu County,80 cases with prostate cancerwere selected and divided into observation group and control group,with 40 cases in each group. Patients in observation group were given transurethral plasmakinetic prostatectomy,and patients in control group were given transurethral resection of prostate. International prostate cancer（ IPSS） score,maximum urinary flow rate,residual urine volume and serum prostate specific antigen（ PSA）,urine tube pulling up time,operation time,intraoperative blood loss and quality of life of two groups were compared. Results Compared with before treatment,IPSS score decreased,the maximum urine flow rate increased,residual urine volume decreased,serum PSA levels of patients in two gorups after treatment were reduced,with statistical difference（ P〈 0. 05）. The difference of IPSS score,maximum urinary flow rate,residual urine volume and serum PSA level in the two groups were not statistically significant（ P〉 0. 05）. Compared with the control group,pulled catheter time of patients in the observation group was shortened,and amount of bleeding during the operation was reduced,with statistical difference（ P〈 0. 05）. There was no significant difference in operation time between the two groups（ P〉 0. 05）.There was no significant difference in the physical function,role function,cognitive function,emotional function,and community functional score between the two groups（ P〈 0. 05）. Conclusions Transurethral plasmakinetic prostatectomy for treatment of prostate cancer has a significant clinical effect,with no significant difference in transurethral resection of the prostate,but it can shorten the time of catheter removal,and reduce the amount of bleeding,can effectively relieve the symptoms of urethral obstruction caused by prostate cancer,improve the quality of
