胶囊内镜综合改良检查法在老年人下消化道疾病诊断中的应用

摘　　要：目的了解胶囊内镜综合改良检查法在老年患者下消化道疾病诊断中的应用。方法 50例需行下消化道内镜检查的老年患者予以胶囊内镜综合改良检查法检测。除有效的肠道准备外,患者在胶囊内镜通过上消化道实施胶囊节能模式,同时以促胃肠动力药、体位变化辅助胶囊进入下消化道并保持良好运行。结果胶囊内镜检查历时260-620min,平均有效记录时间为512.6 min。46例（92.0%）完成胶囊内镜检查,共发现异常40例,其中血管异常9例,黏膜下隆起及新生物7例,黏膜炎症、孤立凹陷性憩室、寄生虫各5例,不明原因出血、疑似炎症性肠病、黏膜红斑各3例,阳性检出率为87.0%（40/46）。结论胶囊内镜检查安全性好,对老年患者消化道疾病有较高的诊断价值。Objective To investigate the application and safety of comprehensive improvement of capsule endoscopy in the diagnosis of lower digestive tract disease in elderly patients. Methods Fifty elderly patients in need of gastrointestinal endoscopy underwent capsule endoscopy. In addition to effective bowel preparation, energy-saving mode was implemented when the capsule endoscopy went through the upper gastrointestinal tract, and prokinetic agents, postural change was used to assist the capsule to go into the lower digestive tract and maintain a good running. Results Capsule endoscopy lasted for 260-620 min, and average effective recording time was 512.6 min. 9 cases of vascular abnormalities（18%）, 7 cases of bulge and submucosal neoformation（14%）, and 5 cases of inflammatory lesions（10%）, parasites（10%） and isolated depressed diverticulum（10%）, respectively. Conclusion Comprehensive improvement of capsule endoscopy is a safe method for elderly patients with lower digestive tract diseases and has a better diagnostic value.
