摘 要:目的对比CT及MR诊断中枢神经细胞瘤的准确率,并对其临床特征进行分析。方法收集2012年1月-2014年11月收治的中枢神经细胞瘤患者36例为研究对象,采用CT和MR检查方法观察患者的影像学特征,并将影像学诊断结果与病理诊断结果进行对比,计算其诊断准确率。结果 CT和MR检查均能清晰显示36例患者的发病位置,最常见的为透明隔和侧脑室壁,也可见于额叶、第四脑室、小脑蚓部及胼胝体内壁。CT检查表现为肿瘤实质呈等密度或稍高密度,增强扫描可呈轻度至中度强化,主要表现为囊性病变、片状钙化及条带状钙化;MR检查表现为肿瘤实质呈等或稍低信号,增强扫描后呈轻度或中度强化,瘤体呈明显蜂窝状或丝瓜瓤样囊变,主要表现为瘤内钙化、血管流空、坏死囊变及出血。与病理诊断结果相比,CT检查确诊为CNC者11例,诊断准确率为30.55%;MR检查确诊为CNC者25例,诊断准确率为70.00%,CT与MR检查均确诊为CNC者18例,诊断准确率为50.00%。MR诊断准确率与CT诊断准确率相比,差异有统计学意义(χ-2=12.56,P=0.035)。结论中枢神经细胞瘤的影像学表现及发病位置具有一定特征性,MR诊断准确率高于CT诊断准确率,临床应根据其影像学特征,选择适当的影像学工具,以提高中枢神经细胞瘤的诊断准确性。Objective To study the diagnostic accuracy of CT and MR on central neurocytoma and analyze the clinical characteristics.Methods 36 patients with central neurocytoma were collected from January 2012 to November 2014 from our hospital.The clinical characteristics of patients were observed by CT and MR method.The results were compared with the pathological diagnosis.The diagnostic accuracy was calculated.Results CT and MR images could clearly show the location of onset of 86 patients in the septum pellucidum,lateral ventricle wall,frontal lobe,the fourth ventricle,cerebellar vermis,and corpus callosum wall.Isodensity or slightly high density of tumor essence was shown by CT examination.Mild or moderate strengthening was shown by enhance scan.Cystic lesion,patchy calcification and banded calcification were the main characteristics.The equisignal signals or slightly lower signals were shown by MR.Mild or moderate strengthening was shown by enhance scan.Calcification within the tumor,empty vessels,necrosis in capsule,and bleeding were the main characteristics.Compared with the pathological diagnosis,the diagnostic accuracy rate of CT examination was 55.81% and diagnostic accuracy of MR images was 75.58%(P〈0.05).Conclusion Central neurocytoma has some characteristics on imaging finding and location of onset.MR diagnosis accuracy of MR is higher than that of CT images.It should be to improve the diagnostic accuracy of central neurocytoma according to its clinical characteristics,combining with the appropriate imaging tools.
