摘 要:目的了解血液净化中心不同护理工作模式对护士工作生活质量的影响。方法采用基本资料问卷及护理工作生活质量量表对血液净化中心持续性肾脏替代治疗(CRRT)组(30人,参与24h待命)和非CRRT组(30人,不参与24h待命)进行调查。结果60名护士工作生活质量总分160.02±20.12;工作生活质量总分及另3个维度得分两组差异无统计学意义(均P>0.05),工作家庭维度得分CRRT组显著低于非CRRT组(P<0.05)。1年内离职率CRRT组显著高于非CRRT组(P<0.05);是否加入CRRT组影响工作家庭维度得分(β′=-0.281,P<0.05)。结论血液净化中心护士工作生活质量偏低,CRRT组工作模式影响护士工作家庭平衡,频繁轮值夜班可导致其离职;建议管理者高度重视此类问题,改进工作机制,从而改善血液净化中心护士的工作生活质量。Objective To investigate the effect of different shift work patterns on quality of work life among nurses in blood purification center.Methods Thirty nurses were required to work 24-hour on-call shifts for continuous renal replacement therapy(CRRT)in blood purification center service and their counterparts(30nurses)did not provide CRRT service without involving this shift pattern.Quality of nursing work life(QNWL)was evaluated in all these nurses.Results The total QNWL score was 160.02±20.12 among nurses in blood purification center.There were no significant differences between the CRRT and non-CRRT groups in the total QNWL score and scores of three subscales(P0.05 for all),whereas the score of work-family balance in the CRRT group was significantly lower than that in the non-CRRT group(P0.05).The 1-year turnover rate was significantly higher in the CRRT group than the non-CRRT group(P0.05).Work-family balance was affected by whether or not the nurse worked in the CRRT group(β′=-0.281,P0.05).Conclusion Quality of work life was relatively poor among nurses in blood purification center.The shift work pattern for CRRT service could influence nurses′work-family balance,and frequent night shifts linked to increased turnover.Managers should pay more attention to these problems,and improve working mechanism,thus to enhance quality of work life among nurses in blood purification center.
