三种不同上部结构应用于下颌种植覆盖义齿的临床疗效比较

摘　　要：目的:比较Locater附着体、球帽式附着体和磁性附着体分别应用于下颌种植覆盖义齿的临床疗效。方法:选取双种植体支持的Locator附着体、球帽式附着体、磁性附着体覆盖义齿治疗的下颌无牙颌患者45例为研究对象进行随访2年,观察3组患者满意度、牙周健康状况和义齿并发症的情况。结果:Locater组和球帽组在义齿咀嚼,舒适,发音及固位和稳定方面优于磁性附着体组,有统计学差异(P<0.05);三组患者的牙周健康状况无统计学差异(P>0.05);三组患者在并发症方面无统计学差异(P>0.05)。结论:采用Locator附着体作为上部结构的下颌种植覆盖义齿对下颌牙槽骨重度吸收老年患者是一种有效的修复方式。Objective：Comparing of the clinical effects of the locater attachment, the ball attachment and the magnetic attachment respectively applied to mandibular implant overdenture. Methods： 45 patients were selected with 2 implant-supported mandibular overdenture, the Locater attachment, the ball attachment and magnetic attachment were applied to the overdenture respectively. The satisfaction of the overdenture, the periodontal condition and the complication of the overdenture were recorded followed up 2 years after the overdenture rehabilitation. Results： The Locater group and the ball group showed advantages to the magnetic group on the aspect of mastication, comfort,pronounce and retention and stability of overdenture, the differences were statistically significant（P〈0.05）; on the aspect of periodontal condition, no significant statistical differences were found（P〈0.05）; the differences among the three groups were not statistically significant regarding the complication of the overdenture（P〈0.05）. Conclusion： mandibular implant overdenture with locater attachment is an effective option for elder patients of severely resorption of mandibular alveolar ridge.
