摘  要:目的观察Nd∶YAG泪道激光成形联合泪道再通管逆行置入术治疗鼻泪管阻塞的疗效。方法回顾性分析2011年2月至2014年12月,鼻泪管阻塞患者189例226只眼,其中单纯性鼻泪管阻塞141例178只眼,慢性泪囊炎48例48只眼。行Nd∶YAG泪道激光成形联合泪道再通管置入术,激光治疗能量为165-1100mJ,置管6个月后拔管并随访6-18个月,观察流泪、流脓、泪道通畅等情况。结果在带管6个月内,溢泪溢脓消失或改善,泪道冲洗畅。拔管后随访6-18个月,治愈117只眼,好转52只眼,总有效率为74.7%。无效57只眼,占25.3%。结论 Nd∶YAG泪道激光成形联合泪道再通管置入术是一种有效的鼻泪管阻塞性疾病的治疗方法。Objective To observe the joint therapeutic effect of Nd : YAG lacrimal laser dacyrocystoplasty and silicon tube stents on nasolacrimal duct obstruction. Methods Totally 189 patients (226 eyes) of nasolacrimal duct obstruction, including 141 cases ( 178 eyes) of simple nasolacrimal duet obstruction and 48 cases (48 eyes) of chronic dacryocystitis, were treated jointly with Nd:YAG lacrimal laser dacyrocystoplasty and silicon tube stents during the period from Feb. 2011 to Dec. 2014. The silicon tube stents were removed 6 months after the opera- tion, and the patients were followed up for at least 6 months. The situation of lacrimal duct, epiphora, purulence and complications of the patients were observed. Results For all the patients, postoperatively improvement or absence of the epiphora and unobstructed lacrimal duct occurred in the first 6 months. The silicon tube stents was removed 6 months after the operation, and the patients were followed up for 6-18 months. By the last follow-up, 117 eyes were cured and 52 improved, indicating a total effectiveness rate of 74. 7%. No effect was observed in 57 eyes, accounting for 25. 3 %. Conclusions Joint application of Nd : YAG lacrimal laser dacyrocystoplasty and silicon tube stents is an effective treatment for nasolacrimal duct obstruction.
