YB-1蛋白在宫颈腺癌组织中的表达及临床意义

摘　　要：目的通过检测Y盒结合蛋白1(YB-1)在正常宫颈、宫颈原位腺癌及宫颈腺癌组织中的表达情况,探讨其与宫颈腺癌发生发展的关系。方法应用免疫组织化学染色法,检测20例正常宫颈组织、28例宫颈原位腺癌组织及73例宫颈腺癌组织中YB-1蛋白的表达。结果⑴YB-1蛋白在三组宫颈组织中均有表达,在正常宫颈和宫颈原位腺癌组织中主要为低表达,阳性表达率分别为25.00%、57.14%;而在宫颈腺癌组织中主要为高表达,阳性表达率为86.30%;三者比较,差异有统计学意义(P<0.05)。⑵YB-1蛋白阳性表达全部定位于细胞质,其中宫颈腺癌组织中出现合并细胞核阳性表达细胞。⑶宫颈腺癌组织中YB-1蛋白表达与FIGO分期(P=0.015)、肿瘤直径(P=0.016)、间质浸润深度(P=0.046)及淋巴结转移(P=0.030)相关;而与患者年龄(P=0.067)、有无脉管癌栓(P=0.059)及宫旁浸润(P=0.587)无关。结论 YB-1蛋白过表达与宫颈腺癌的发生发展及淋巴结转移相关,通过检测YB-1蛋白在宫颈腺癌组织中的表达情况,有望为宫颈腺癌的诊断、治疗及预后评估提供一个新指标。Objective By detecting expression of Y box binding protein 1（YB-1） in the normal cervix,cervical adenocarcinoma in situ and cervical adenocarcinoma tissues,explore its relationship with the development of cervical adenocarcinoma. Methods Immunohistochemistry was used to detect expression of YB-1 protein in 20 cases of normal cervical tissue,28 cases of cervical adenocarcinoma in situ tissue and 73 cases of cervical adenocarcinoma. Results ⑴In three groups of cervical tissues,YB-1 protein was all expressed,but in normal cervix and cervical adenocarcinoma in situ was mainly low expression,the rates of positive expression were 25.00% and 57.14% respectively;while in cervical adenocarcinoma mainly high expression,positive expression rate was 86.30%;there was significant difference among three groups（P0.05）. ⑵YB-1 protein expression all localized in the cytoplasm,but in cervical adenocarcinoma appeared cell nucleus expressing. ⑶ the expression of YB-1 protein was related to FIGO stage（P=0.015）,tumor diameter（P=0.016）,depth of stromal invasion（P=0.046） and lymph node metastasis（P=0.030）;but was not related to the patient＇s age（P=0.067）,vascular invasion（P=0.059） and parametrial invasion（P=0.587）. Conclusion YB-1 protein expression was related to the development and lymph node metastasis of cervical cancer,and the expression of YB-1 protein in adenocarcinoma of the uterine cervix is expected to be a new index for early diagnosis and prognosis assessment.
